摘　　要：目的 分析乙肝肝癌患者甲胎蛋白(AFP)、甲胎蛋白异质体(AFP-L3)、热休克转录因子1(HSF1)、热休克蛋白27(HSP27)表达水平及与预后的关系。方法 选取2021年6月至2022年6月接收的30例乙肝肝癌患者为观察组,35例乙肝非肝癌患者为对照1组,35名健康体检者为对照2组。检测所有研究对象的血清AFP、AFP-L3、HSF1 mRNA及HSP27表达水平,分析其与预后的关系。结果 观察组的AFP、AFP-L3、HSF1 mRNA及HSP27水平高于对照1组、对照2组(P<0.05)。AFP、AFP-L3、HSF1 mRNA及HSP27高表达乙肝肝癌患者的TNM分期Ⅲ~Ⅳ期、肿瘤≥5 cm、低分化、淋巴结转移占比高于低表达患者(P<0.05)。AFP、AFP-L3、HSF1 mRNA、HSP27高表达是乙肝肝癌患者预后不良的危险因素(P<0.05)。结论 乙肝肝癌患者血清AFP、AFP-L3、HSF1、HSP27均呈现出高表达现象,且其是预后不良的危险因素。Objective To analyze the expression levels of alpha fetoprotein(AFP),alpha fetoprotein-L3(AFP-L3),heat shock transcription factor 1(HSF1)and heat shock protein 27(HSP27)in patients with hepatitis B liver cancer and their relationship with prognosis.Methods A total of 30 patients with hepatitis B liver cancer received from June 2021 to June 2022 were selected as observation group,35 hepatitis B patients without liver cancer were selected as control group 1,and 35 healthy subjects were selected as control group 2.The expression levels of serum AFP,AFP-L3,HSF1 mRNA and HSP27 of all subjects were detected,and their relationship with prognosis were analyzed.Results The levels of AFP,AFP-L3,HSF1 mRNA and HSP27 in the observation group were higher than those in the control group 1 and the control group 2(P<0.05).The proportions of TNM stageⅢ-Ⅳ,tumor≥5 cm,poor differentiation and lymph node metastasis in hepatitis B liver cancer patients with high expression of AFP,AFP-L3,HSF1 mRNA and HSP27 were higher than those in patients with low expression(P<0.05).The high expression of AFP,AFP-L3,HSF1 mRNA and HSP27 were risk factors for poor prognosis in patients with hepatitis B liver cancer(P<0.05).Conclusion Serum AFP,AFP-L3,HSF1 and HSP27 in patients with hepatitis B liver cancer show high expression,which were risk factors for poor prognosis.
