摘  要:目的:探讨集成磁共振成像(SyMRI)技术对足月新生儿脑髓鞘发育评估的可行性和应用价值。方法:回顾性纳入2020年9月-2023年10月在我院成功完成常规MRI和SyMRI检查的正常足月新生儿80例,将其按照胎龄分为三组,A组胎龄为37~38W,B组胎龄为39~40W,C组胎龄为41~42W。手动勾画感兴趣区(ROI),应用SyMRI后处理软件分别测量各ROI的T1、T2值并进行统计学分析。采用配对样本t检验比较组内左右大脑半球同一ROI以及组内不同ROI的T1、T2值差异有无统计学意义;使用单因素方差分析或Kruskal-Wallis H检验比较组间各ROI的T1、T2值差异有无统计学意义。结果:组内各ROI的T1、T2值不同,两两比较均发现T1、T2值在内囊后肢低于前肢,胼胝体压部低于膝部,枕叶白质低于额叶白质,位于中央的内囊后肢的T1、T2值低于周围白质,差异均有统计学意义(P均<0.05)。其中内囊后肢的弛豫值最短,额叶白质的弛豫值最长。对组间各ROI的T1、T2值进行比较,三组间T1值在内囊前肢差异有统计学意义(P<0.05),三组间T2值在内囊前肢、后肢、胼胝体压部、额叶白质、枕叶白质差异有统计学意义(P<0.05)。结论:SyMRI技术所获取的T1、T2弛豫值可以定量评估足月新生儿脑髓鞘形成和大脑的成熟度,不同区域T1、T2值能够反映髓鞘形成时间的差异。Objective:To explore the feasibility and application value of synthetic MRI(syMRI)in evaluating myelin development in the cerebral white matter of full-term neonates.Methods:A retrospective study was conducted,including 80 full-term neonates who underwent both routine MRI and SyMRI between September 2020 and October 2023.The neonates were divided into three groups based on gestational age:Group A(37~38 weeks),Group B(39~40 weeks),and Group C(41~42 weeks).Regions of interest(ROIs)were manually delineated,and SyMRI post-processing software was used to measure the T1 and T2 values for each ROI,followed by statistical analysis.Paired sample t-tests were applied to compare the T1 and T2 values of the same ROI in the left and right hemispheres,as well as to compare different ROIs within each group for statistical differences.One-way analysis of variance(ANOVA)or Kruskal-Wallis H tests were used to compare the T1 and T2 values of the ROIs between groups.Results:Significant differences in T1 and T2 values were found within the same gestational age group.Specifically,the T1 and T2 values in the posterior limb of the internal capsule(PLIC)were lower than those in the anterior limb of the internal capsule(ALIC).Additionally,the T1 and T2 values in the compressed region of the corpus callosum(CCS)were lower than those in the non-compressed region(CCG),and the T1 and T2 values in the white matter of the occipital lobe were lower than those in the frontal lobe.The T1 and T2 values in the central PLIC were lower than those in the surrounding white matter(P<0.05 for all comparisons).Among the regions,the relaxation values of the PLIC were the shortest,while the relaxation values of the frontal lobe white matter were the longest.When comparing T1 and T2 values across different gestational age groups,statistically significant differences(P<0.05)were observed in the T1 values of the ALIC.For the T2 values,significant differences(P<0.05)were found in the ALIC,PLIC,CCS,frontal lobe white matter,and occipital lobe white matter.Conclus
