摘  要:目的:探讨常规超声、超声造影及超声弹性成像参数与甲状腺乳头状癌半乳糖凝集素-3(Galectin-3)、死骨片1(sequestosome 1,SQSTM1)表达水平及预后的相关性。方法:搜集接受手术治疗的92例甲状腺乳头状腺癌患者,术中取患者癌组织及癌旁正常组织。术前对所有患者进行常规超声、超声造影及超声弹性成像检查,观察并记录病灶最大径、形态、边界、纵横比、微钙化、血流信号分级,弹性成像参数弹性最大值(Emax)、弹性最小值(Emin)、弹性均值(Emean)、弹性标准差(Emen),超声造影增强方式、增强程度、增强均匀性、达峰时间、峰值强度和曲线下面积。采用免疫组织化学法对所有甲状腺乳头状癌组织和癌旁正常组织中Galectin-3、SQSTM1蛋白表达水平进行检测。术后对所有患者随访3年,观察无进展生存期,根据是否发生进展,将患者分为预后不良组和预后良好组。结果:甲状腺乳头状癌组织中Galectin-3、SQSTM1高表达水平的比例显著高于癌旁正常组织,差异有统计学意义(P<0.05)。Galectin-3高表达患者的肿瘤最大径、出现微钙化、血流信号2~3级、超声造影增强不均匀的比例显著高于低表达患者,峰值强度、 Emax、Esd显著高于低表达患者,差异有统计学意义(P<0.05)。SQSTM1高表达患者出现微钙化、血流信号2~3级、增强不均匀的比例显著高于低表达患者,达峰时间显著低于低表达患者,峰值强度、Emax、Emean、Esd显著高于低表达患者,差异有统计学意义(P<0.05)。预后不良患者出现血流信号2~3级、增强不均匀的比例显著高于预后良好患者,峰值强度、Emax、Esd显著高于预后良好患者,差异有统计学意义(P<0.05)。多因素logistic回归分析结果表明,肿瘤最大径≥1 mm、血流信号2~3级、较大的峰值强度、Emax和Esd是Galectin-3高表达的独立危险因素(P<0.05)。血流信号2~3级、增强不均匀、较大的峰值强度、Emax和Objective:The purpose of this study was to investigate the correlation of conventional ultrasound,contrast-enhanced ultrasound and elastography parameters with the expression levels of Galectin-3 and SQSTM1 and prognosis in papillary thyroid carcinoma.Methods:A total of 92 patients with papillary thyroid adenocarcinoma who received surgical treatment were collected.Cancer tissues and adjacent normal tissues were obtained during the operation.Conventional ultrasound,contrast-enhanced ultrasound and ultrasound elastography were performed on all patients before surgery.The maximum diameter,shape,boundary,aspect ratio,microcalcification and blood flow signal classification of lesions,elastic maximum value(Emax),elastic minimum value(Emin),elastic mean value(mean)and elastic standard deviation(Emen)of elastic imaging parameters,and enhancement method,enhancement degree,enhancement uniformity,peak time,peak intensity and area under the curve of contrast-enhanced ultrasound were all observed and recorded.The expression levels of Galectin-3 and SQSTM1 proteins in papillary thyroid carcinoma and para-carcinoma normal tissues were detected by immunohistochemistry.All patients were followed up for 3 years after surgery to observe progression-free survival.According to whether progression occurred,the patients were divided into poor prognosis group and good prognosis group.Results:The high expression levels of Galectin-3 and SQSTM1 in papillary thyroid carcinoma tissues were significantly higher than those in para-carcinoma normal tissues,and the difference was statistically significant(P<0.05).The proportions of tumor maximum diameter,microcalcification,grade 2~3 blood flow signal,and uneven enhancement of contrast-enhanced ultrasound in patients with high expression of Galectin-3 were significantly higher than those in patients with low expression,and the peak intensity,Emax,and Esd of patients of high expression of Galectin-3 were significantly higher than those in patients with low expression,with statistical significanc
