银杏二萜内酯葡胺注射液结合经筋针刺对脑卒中偏瘫患者的疗效及对cAMP、cGMP及BDNF的作用分析

摘　　要：目的探讨银杏二萜内酯葡胺注射液结合经筋针刺对脑卒中偏瘫患者的疗效及对血清环磷酸腺苷(cyclic adenosine phosphate,cAMP)、环磷酸鸟苷(cyclic guanosine phosphate,cGMP)、脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)水平的影响。方法选择2022年3月至2024年3月我院脑卒中偏瘫患者150例,按照随机数字表法分为观察组(常规治疗+银杏二萜内酯葡胺注射液+经筋针刺)和对照组(常规治疗+银杏二萜内酯葡胺注射液),每组75例,两组均持续治疗8周。比较两组临床疗效、血清cAMP、cGMP、BDNF、美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)、Fugl-Meyer运动功能评分量表(Fugl-Meyer motor function assessment,FMA)评分及中医证候积分变化。结果观察组总有效率高于对照组(88.00%vs 70.67%)(P<0.05)。观察组血清cAMP、BDNF均高于对照组(P<0.05),cGMP低于对照组(P<0.05)。观察组NIHSS评分低于对照组(P<0.05),FMA评分高于对照组(P<0.05)。观察组半身不遂、口舌歪斜、言语謇涩证候积分均低于对照组(P<0.05)。结论银杏二萜内酯葡胺注射液联合经筋针刺治疗脑卒中偏瘫效果显著,两者具有协同作用,可调节血清cAMP、cGMP、BDNF水平,促进功能恢复。Objective To investigate the therapeutic effect of ginkgo biloba diterpene lactone meglumine injection combined with acupuncture through tendons on hemiplegia patients with stroke and its effects on cyclic adenosine phosphate(cAMP),cyclic guanosine phosphate(cGMP)and brain-derived neurotrophic factor(BDNF)in serum.Methods A total of 150 stroke patients with hemiplegia in our hospital from March 2022 to March 2024 were selected and divided into observation group(conventional treatment+ginkgo biloba diterpenoid lactone meglumine injection+acupuncture through tendons)and control group(conventional treatment+ginkgo biloba diterpenoid lactone meglumine injection)according to random number table method,75 cases in each group,and both groups were treated for 8 weeks.The clinical efficacy,serum cAMP,cGMP,BDNF,and scores of National Institute of Health stroke scale(NIHSS),Fugl-Meyer motor function assessment(FMA)and traditional Chinese medicine syndrome scores were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(88.00%vs 70.67%,P<0.05).Serum cAMP and BDNF in observation group were higher than those in control group(P<0.05),and cGMP was lower than that in control group(P<0.05).The NIHSS score of the observation group was lower than that of the control group(P<0.05),and the FMA score was higher than that of the control group(P<0.05).The syndrome scores of hemiplegia,tongue deviation,and speech astringency syndrome in the observation group were lower than those in the control group(P<0.05).Conclusion Ginkgo biloba diterpene lactone meglumine injection combined with acupuncture through tendons has a significant effect in the treatment of hemiplegia after stroke.The two have a synergistic effect,which can regulate the levels of serum cAMP,cGMP and BDNF,and promote functional recovery.
