基层医院护士对院内卒中识别和救治流程的知晓程度及培训需求分析

摘　　要：目的了解基层医院护士对院内卒中(in-hospital stroke,IHS)识别和救治流程的知晓程度及培训需求,并分析其影响因素。方法2023年3月采用护士IHS速识别和救治流程的知晓程度及培训需求问卷对赣州市基层医院在职护士进行调查。问卷内容包括一般资料、IHS快速识别和救治流程的知晓程度、IHS快速识别和救治流程相关知识及培训需求共24个条目。结果共回收来自85家单位的1236份有效问卷,基层护士对IHS识别和救治流程知晓程度得分为(18.59±3.98)分。多元线性回归分析显示,职称、脑卒中患者接触史、所在科室IHS发生的频率是知晓程度得分的影响因素(P<0.05)。在IHS快速识别和急救知识方面,有93名(7.50%)护士知晓静脉溶栓时间窗4.5 h内,780名(63.10%)能选择“通过平衡、眼、面、上肢、语言、时间(balance-eyes-face-arms-speech-time,BEFAST)卒中识别量表”判断,745名(60.30%)能通过“卒中120”识别。在IHS快速识别与救治的培训需求方面,1208名(97.7%)护士非常愿意学习相关知识,被调查者期望通过科室或医院培训(66.80%)、现场救治或模拟训练(65.70%)等多种形式接受相关卒中识别及救治培训,提升其在快速识别(84.60%)、识别后急救处理(84.20%)等相关能力。结论基层医院护士对IHS识别及救治流程知晓程度偏低,缺乏相关知识的培训。未来应开展常态化培训,强化基层护士对IHS防控重要性的认知,以提升IHS识别率和改善基层医院服务水平。Objective To understand the awareness and training needs of nurses on stroke identification and treatment process of in-hospital stroke(IHS)in primary hospitals,and to analyze its influencing factors.Methods In March 2023,a questionnaire of awareness and training needs of IHS rapid identification and treatment process on nurses was used to investigate nurses in primary hospitals in Ganzhou.A total of 24 items included in the questionnaire,which were general data,awareness of IHS rapid identification and treatment process,and the related knowledge and training needs of IHS rapid identification and treatment process.Results A total of 1,236 valid questionnaires were collected from 85 primary hospitals,and the scores of awareness of IHS identification and treatment process of primary nurses was(18.59±3.98)points.Multiple linear regression analysis showed that professional title,contact history of stroke patients,and the frequency of IHS in the department were the influencing factors of the awareness score(P<0.05).In terms of IHS rapid identification and first aid knowledge,93(7.50%)nurses knew about the intravenous thrombolysis time window within 4.5 h,780(63.1%)nurses could identify stroke through"the balance-eyes-face-arms-speech-time(BEFAST)stroke identification scale",and 745 people(60.30%)were identified by"stroke 120".In terms of training needs of IHS rapid identification and treatment,1208(97.7%)nurses were very willing to learn relevant knowledge,and the respondents expected to receive relevant stroke identification and treatment training through department or hospital training(66.80%),on-site treatment or simulation training(65.70%),so as to improve their ability in rapid identification(84.60%)and post-identification emergency treatment(84.20%).Conclusions Nurses in primary hospitals has a low awareness of IHS identification and treatment process,and lack of relevant knowledge training.In the future,regular IHS training should be carried out to strengthen the grassroots nurses'cognition of the importance o
