摘 要:目的探讨能谱电子计算机断层扫描(CT)在胃癌(GC)、胃间质瘤(GIST)鉴别诊断中的临床价值。方法回顾性分析2021年1月至2023年5月新乡医学院第四临床学院收治的130例胃部常见肿瘤患者的临床资料,根据病理检查结果将其分为GC组(89例)和GIST组(41例),其中GC组根据分化程度分为低分化组(42例)和中、高分化组(47例)。所选患者均接受能谱CT检查,比较GC组和GIST组动脉期(AP)、静脉期(VP)下的碘浓度(IC)、水浓度(WC)、标准化碘含量(NIC)值,对比GC组和GIST组、低分化组和中、高分化组AP、VP 40~140 keV不同单能量下的CT值及能谱曲线斜率λ。结果GC组VP下的IC、NIC值均高于GIST组(P<0.05)。GC组AP、VP 40 keV、90 keV、140 keV下的CT值均高于GIST组(P<0.05);AP、VP 40~140 keV下的能谱曲线斜率λ均高于GIST组(P<0.05)。低分化组AP、VP 40 keV、90 keV、140 keV下的CT值均高于中、高分化组(P<0.05);AP、VP 40~140 keV下的能谱曲线斜率λ均高于中、高分化组(P<0.05)。结论GC和GIST能谱多参数定量有助于鉴别诊断GC和GIST。Objective To investigate the clinical value of energy dispersive electronic computed tomography(CT)in the differential diagnosis of gastric cancer(GC)and gastric stromal tumors(GIST).Methods The clinical data of 130 patients with common gastric tumors admitted to the Fourth Clinical College of Xinxiang Medical College from January 2021 to May 2023 were retrospectively analyzed.They were divided into the GC group(89 cases)and the GIST group(41 cases)according to the pathological examination results,among which GC group was divided into the low differentiation group(42 cases)and the moderate and high differentiation group(47 cases)according to differentiation degree.All selected patients underwent energy spectrum CT examination,and the iodine concentration(IC),water concentration(WC),and standardized iodine content(NIC)values in the arterial phase(AP)and venous phase(VP)of the GC group and the gastric GIST group were compared.The CT values and energy spectrum curve slopeλin AP,VP 40-140KeV under different single energy of the GC group and GIST group,as well as the low differentiation group and medium and high differentiation group were compared.Results The IC and NIC values under VP in the GC group were higher than those in the gastric GIST group(P<0.05).The CT values at AP,VP 40 keV,90 keV,and 140 keV in the GC group were higher than those in the gastric GIST group(P<0.05).The energy spectrum curve slopeλat AP and VP 40-140 keV were higher than those in the gastric GIST group(P<0.05).The CT values at AP,VP 40 keV,90 keV,and 140 keV in the low differentiation group were higher than those in the medium and high differentiation groups(P<0.05).The energy spectrum curve slopeλat AP and VP 40-140 keV were higher than those in the medium and well differentiated groups(P<0.05).Conclusion Multi parameter quantification of GC and GIST spectra was helpful in distinguishing and diagnosing GC and GIST.
