摘 要:目的检测乳腺癌组织中DNA拓扑异构酶Ⅱ(TOP2A)蛋白的表达,并分析TOP2A蛋白表达水平与乳腺癌蒽环紫杉类新辅助化疗后完全缓解率(pCR)的关系。方法收集宁夏医科大学总医院肿瘤医院2010年1月-2019年11月经组织病理证实的289例女性新辅助化疗的乳腺癌患者,所有患者均行蒽环紫杉类新辅助化疗。采用免疫组化方法检测TOP2A蛋白的表达,并采用受试者工作曲线(ROC曲线)确定其Cut-off值。结果TOP2A蛋白表达率在0~99%,中位数为30%,Cut-off值为30%,其中TOP2A蛋白高表达(>30%)169例(58.48%)、低表达(≤30%)120例(41.52%)。TOP2A蛋白高表达pCR率为34.17%(41/120),TOP2A蛋白低表达的pCR率为18.34%(31/169),组间比较,差异有统计学意义(P<0.05)。单因素分析显示,T分期、TOP2A、Ki-67及ER、PR表达与pCR率相关(P<0.05)。Logistic回归分析显示,T分期与TOP2A蛋白高表达是乳腺癌蒽环联合紫杉类新辅助化疗疗效pCR率的独立预测因素(P<0.05)。结论乳腺癌组织中TOP2A蛋白表达水平存在差异,TOP2A蛋白表达率>30%是乳腺癌患者蒽环紫杉类药物新辅助化疗后pCR率的独立预测因子。Objective To detect the expression of DNA topoisomeraseⅡ(TOP2A)protein in breast cancer tissues,and to analyze the relationship between TOP2A protein expression level and pathologic complete remission rate(pCR)after neoadjuvant chemotherapy with anthracycline taxanes in breast cancer.Methods A total of 289 female breast cancer patients with neoadjuvant chemotherapy confirmed by histopathology in the Tumor Hospital,General Hospital of Ningxia Medical University from January 2010 to November 2019 were collected.All patients underwent anthracycline taxane neoadjuvant chemotherapy.The expression of TOP2A protein was detected by immunohistochemistry,and the Cut-off value was determined by receiver operating curve(ROC curve).Results The expression rate of TOP2A protein was 0-99%,the median was 30%,and the Cut-off value was 30%.Among them,169 patients(58.48%)had high expression of TOP2 A protein(>30%)and 120 patients(41.52%)had low expression(≤30%).The pCR rate of high expression of TOP2A protein was 34.17%(41/120),and the pCR rate of low expression of TOP2A protein was 18.34%(31/169),the difference between the two groups was statistically significant(P<0.05).Univariate analysis showed that T stage,TOP2A,Ki-67,ER and PR expression were correlated with pCR rate(P<0.05).Logistic regression analysis showed that T stage and high expression of TOP2A protein were independent predictors of pCR rate of anthracycline combined with taxane neoadjuvant chemotherapy for breast cancer(P<0.05).Conclusion The expression level of TOP2A protein in breast cancer tissues is different.TOP2A protein expression rate>30%is an independent predictor of pCR rate after neoadjuvant chemotherapy with anthracycline taxanes in breast cancer patients.
