摘 要:目的探讨过敏原特异性抗体IgE(specific IgE,sIgE)和食物特异性抗体IgG4(specific IgG4,sIgG4)在过敏性疾病诊疗中的临床意义。方法以2022年5月19日—2023年11月14日在三明市第二医院就诊的1232例过敏性疾病患者为试验对象,按症状分为皮肤过敏组(湿疹、荨麻疹和特应性皮炎,n=815)、耳鼻喉过敏组(鼻炎和鼻窦炎,n=214)和呼吸道过敏组(咳嗽、哮喘和支气管炎,n=203),联合检测血清sIgE和sIgG4。结果1232例过敏性患者总阳性率为85.96%,sIgE总阳性率47.32%,sIgG4阳性率75.41%;耳鼻喉过敏组阳性率高于皮肤过敏组和呼吸道过敏组,差异均有统计学意义(P<0.05),且sIgE和sIgG4均阳性者占比最高。耳鼻喉过敏组中户尘螨/粉尘螨、屋尘、狗毛皮屑、鸡蛋白、牛奶、牛肉、虾的sIgE阳性率高于皮肤过敏组和呼吸道过敏组,鸡蛋、牛奶、牛肉、虾、大豆、小麦、鸡肉、鳕鱼、蘑菇的sIgG4阳性率高于皮肤过敏组和呼吸道过敏组,差异均有统计学意义(P<0.05)。皮肤过敏组、耳鼻喉过敏组和呼吸道过敏组不同过敏原种类数量sIgG4检测阳性占比构成比较,差异有统计学意义(P<0.05),其中皮肤过敏组sIgG4检测阳性者占比最高的为1种过敏原者(41.28%),呼吸道过敏组占比最高的为1种过敏原者(39.62%),耳鼻喉过敏组占比最高的为3种以上过敏原者(32.83%)。结论sIgE和sIgG4联合检测对防治过敏性疾病有重要的临床指导意义。Objective To explore the clinical significance of allergen-specific IgE antibodies(sIgE)and food-specific IgG4 antibodies(sIgG4)in the diagnosis and treatment of allergic diseases.Methods A total of 1232 allergic patients who attended the Second Hospital of Sanming from May 19th,2022 to November 14th,2023 as test subjects.They were divided into skin allergy group(eczema,urticaria,and atopic dermatitis,n=815),ear,nose,and throat allergy group(rhinitis and sinusitis,n=214),and respiratory allergy group(cough,asthma,and bronchitis,n=203)according to symptoms.Serum sIgE and sIgG4 were jointly detected.Results Among the 1232 allergic patients,the total positive rate was 85.96%,the total positive rate of sIgE was 47.32%,and the positive rate of sIgG4 was 75.41%.The positive rate of the ear,nose,and throat allergy group was higher than that of the skin allergy group and the respiratory allergy group,and the differences were statistically significant(P<0.05),and possessed the highest percentage of simultaneous positive for both sIgE and sIgG4.The sIgE positivity rates of Dermatophagoides pteronyssinus/Dermatophagoides farina,house dust,dog hair dander,egg white,milk,beef,and shrimp in the ear,nose,and throat allergy group were higher than those in the skin allergy group and the respiratory allergy group,and the sIgG4 positivity rates of egg,milk,beef,shrimp,soybeans,wheat,chicken,cod,and mushrooms were higher than those in the skin allergy group and the respiratory allergy group,and the differences were statistically significant(P<0.05).The proportion of sIgG4 positive detections for different types of allergens in the skin allergy group,the ear,nose,and throat allergy group,and the respiratory allergy group was compared,and the difference was statistically significant(P<0.05).Among them,the highest proportion of sIgG4 positive detections in the skin allergy group was for one allergen(41.28%),the highest proportion in the respiratory allergy group was for one allergen(39.62%),and the highest proportion in the ear,nose,an
