摘 要:单侧髁突肥大(unilateral condylar hypertrophy,UCH)是一类发育性颌骨不对称疾病,其病因及发病机制目前尚不明确。主要表现为单侧髁突过度生长、面部进行性偏斜、咬合紊乱、颞下颌关节功能障碍等,患者常因面部不对称就诊。本文报告1例UCH患者,该患者左侧髁突肥大,偏颌畸形明显影响咬合功能。在行左侧髁突高位切除、上颌骨LefortⅠ型及双侧下颌矢状劈开术后患者面型及咬合功能改善。在诊疗过程中利用软件进行虚拟手术设计,术中使用3D?板,缩短手术时间,手术效果良好。Unilateral condylar hypertrophy(UCH)is a kind of developmental asymmetry of jaw bone diseases,its etiology and pathogenesis are not clear.The main manifestations are unilateral condylar overgrowth,progressive facial deviation,occlusal disorder,temporomandibular joint dysfunction,etc.Patients often seek treatment for facial asymmetry.This paper reported a case of UCH.The patient had left condylar hypertrophy and mandibular deformity which significantly affected occlusal function.The patient's facial shape and occlusal function improved after left high condylectomy,maxillary Le Fort I osteotomy and bilateral sagittal split ramus osteotomy.In the diagnosis and treatment process,software was used to carry out virtual surgical design,and 3D occlusal splints was used during operation,which shortened the operation time and achieved good surgical results.
