高频超声联合血清CA125、CA199、CYFRA21-1对乳腺肿块的鉴别诊断价值

摘　　要：目的:探讨高频超声联合血清糖类抗原125(CA125)、糖类抗原199(CA199)、细胞角蛋白19片段(CYFRA21-1)对乳腺肿块的鉴别诊断价值。方法:选取2021年6月—2023年4月信阳市妇幼保健院收治的120例乳腺肿块患者作为研究对象,根据手术或病理活检结果将其分为恶性组(86例)、良性组(34例)。比较两组患者高频超声诊断乳腺肿块结果,血清CA125、CA199、CYFRA21-1水平及其联合诊断结果,并分析其单独及联合诊断价值。结果:相较于良性组,恶性组患者乳腺肿块多呈不规则形状,且存在钙化现象,肿块内部呈不均匀回声,边界不清晰,纵横比>1,差异均有统计学意义(χ^(2)=91.962、79.099、53.861、72.881、90.133,P<0.05)。恶性组患者CA125、CA199、CYFRA21-1均明显高于良性组,差异均有统计学意义(t=27.986、18.877、15.019,P<0.05)。相较于单独检测,联合诊断的灵敏度、准确度均较高,漏诊率较低,差异均有统计学意义(χ^(2)=17.425、21.217、17.425,P<0.05)。结论:高频超声、CA125、CA199、CYFRA21-1联合诊断可显著提升诊断灵敏度,降低漏诊率,可为临床诊断乳腺肿块提供可靠依据。Objective:To investigate the value of high frequency ultrasound combined with serum CA125,CA199 and CYFRA21-1 in the differential diagnosis of breast mass.Methods:A total of 120 patients with breast mass admitted to the hospital from June 2021 to April 2023 were selected and divided into malignant group(86 cases)and benign group(34 cases)with surgical or pathological biopsy as the gold standard.The results of high-frequency ultrasound diagnosis of breast mass were statistically analyzed.The serum levels of CA125,CA199,CYFRA21-1 and their combined diagnostic results were compared between the two groups,and their diagnostic value was analyzed separately and in combination.Results:Compared with the benign group,the malignant group had irregular shape,unclear boundary,calcification,uneven echo and aspect ratio>1,with statistically significant difference(χ^(2)=91.962,79.099,53.861,72.881,90.133;P<0.05).Serum levels of CA125,CA199 and CYFRA21-1 in malignant group were significantly higher than those in benign group(t=27.986,18.877,15.019;P<0.05).The sensitivity and accuracy of HF ultrasound combined with serum CA125,CA199 and CYFRA21-1 were higher than that of single diagnosis,and the rate of missed diagnosis was lower than that of single diagnosis(χ^(2)=17.425,21.217,17.425;P<0.05).Conclusion:The combined diagnosis of high frequency ultrasound,serum CA125,CA199,CYFRA21-1 can significantly improve the diagnostic sensitivity,reduce the rate of missed diagnosis,and provide reliable basis for clinical diagnosis of breast mass.
