摘　　要：目的:观察艾司奥美拉唑联合蛇毒凝血酶治疗急性上消化道出血的效果。方法:选取肇庆市高要区人民医院急诊科2022年3月~2024年3月70例急性上消化道出血患者随机分为两组(n=35)。对照组采用艾司奥美拉唑治疗,观察组采用艾司奥美拉唑联合蛇毒凝血酶治疗。评估两组治疗前后各项指标。结果:治疗后,与对照组相比,观察组血压稳定时间、止血时间、腹部缓解时间均更短(P<0.05),总有效率更高,差异有统计学意义(P<0.05);肿瘤坏死因子(TNF-α)、超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)检测水平均更低,差异有统计学意义(P<0.05);凝血酶时间(TT)、血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)指标均更短,差异有统计学意义(P<0.05);纤维蛋白原(FIB)指标更高,差异有统计学意义(P<0.05)。治疗后,两组不良反应发生率差异无统计学意义(P>0.05)。结论:上消化道出血患者应用艾司奥美拉唑联合蛇毒凝血酶进行治疗,能够有效提升疗效,对患者的止血时间进行缩短,调节患者的凝血功能指标,消除炎性反应,具有较高的安全性。Objective To observe the effect of esomeprazole combined with snake venom thrombin in the treatment of acute upper gastrointestinal bleeding.Method A total of 70 patients with acute upper gastrointestinal bleeding from March 2022 to March 2024 were selected and randomly divided into two groups(n=35),the control group was treated with esomeprazole,and the observation group was treated with esoprazole combined with snake venom thrombin.The indicators before and after treatment were evaluated in both groups.Results After treatment,compared with the control group,the time of blood pressure stabilization,hemostasis and abdominal remission in the observation group were shorter(P<0.05),the total effective rate of patients in the observation group was higher than that in the control group(P<0.05).The levels of TNF-α,hs-CRP and IL-6 in the observation group were lower(P<0.05).The TT,PT and APTT indexes in the observation group were shorter(P<0.05),and the FIB index in the observation group was higher(P<0.05).After treatment,there was no difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of patients with upper gastrointestinal bleeding with esoprazole combined with snake venom thrombin can effectively improve the curative effect,shorten the hemostasis time of patients,regulate the coagulation function of patients,eliminate inflammatory reactions,and have high safety.
