基于“久病入络”理论探究中风后遗症期病机及治疗

摘　　要：早在《黄帝内经》就已经对中风病有了初步认识,并根据其症状命名为“薄厥”“偏枯”等。《金匮要略》提出:“夫风之为病,当半身不遂……中风使然。”首次详细记录了中风病名,并创立相关基本术语,描述精准简练,一直被沿用至今,如“半身不遂”“口角歪斜”“言语不利”“神志异常”等。可见,经典古籍对中医治疗中风后遗症有较为详细的论述,构建了中风诊疗体系,切合临床,为后世对中风治疗研究提供了借鉴和指导作用。文章通过对中医经典、病因病机、临床治疗的归纳总结,探讨中风后遗症期与“久病入络”的内在相关性,体现“络脉以通为用”的特色辨治方略,以期更好应用中医药疗法治疗中风后遗症。“Huangdi Neijing”had a preliminary understanding of stroke and named it“Bojue”and“Pianku”according to its symptoms.The Golden Chamber Synopsis proposed:“When a person has a wind disease,hemiplegia should occur...It is caused by a stroke.”It was the first time that the name of a stroke was recorded in detail,and related basic terms were created,with precise and concise descriptions that have been used to this day,such as“hemiplegia”,“crooked corners of the mouth”,“unfavorable speech”,and“abnormal consciousness”.It can be seen that the classic ancient books have a relatively detailed discussion on the treatment of stroke sequelae by traditional Chinese medicine,and have constructed a stroke diagnosis and treatment system that is suitable for clinical practice,providing reference and guidance for future research on stroke treatment.This article discusses the intrinsic correlation between the sequelae of stroke and“long-term disease entering the collaterals”through the summary of traditional Chinese medicine classics,etiology and pathogenesis,and clinical treatment,and reflects the characteristic treatment strategy of“collaterals are used for unblocking”,in order to better apply traditional Chinese medicine to treat stroke sequelae.
