皮肤鳞状细胞癌组织中SIRT3、PDCD4的表达及其与临床病理特征的关系

摘　　要：目的探究皮肤鳞状细胞癌组织中SIRT3、PDCD4的表达及其与临床病理特征的关系。方法收集209例皮肤鳞状细胞癌患者临床资料(性别、年龄、病理分化分型、肿瘤直径、淋巴结转移)。收集癌组织及癌旁组织标本,采用qRT-PCR测定组织中SIRT3、PDCD4 mRNA水平,比较皮肤鳞状细胞癌组织与癌旁组织的SIRT3、PDCD4 mRNA水平,并进一步分析SIRT3、PDCD4表达与皮肤鳞状细胞癌临床病理特征的关系。采用ROC曲线分析SIRT3、PDCD4表达对皮肤鳞状细胞癌患者淋巴结转移的预测价值。结果与癌旁组织相比,皮肤鳞状细胞癌组织SIRT3水平显著升高,PDCD4表达水平明显降低,差异有统计学意义(P<0.001)。不同性别患者的SIRT3、PDCD4表达水平比较,差异无统计学意义(P>0.05);但不同年龄、病理分化分型、肿瘤直径、淋巴结转移的患者SIRT3、PDCD4表达水平比较,差异有统计学意义(P<0.05)。对淋巴结转移的预测方面,SIRT3的预测阈值为1.025,灵敏度69.4%,特异度78.2%;PDCD4的预测阈值为0.478,灵敏度、特异度较差。SIRT3、PDCD4以及二者联合的AUC面积分别为0.766、0.675、0.809,二者联合的AUC面积最大,灵敏度相对单一指标较高,预测效能相对较好。结论皮肤鳞状细胞癌组织中SIRT3呈高表达,PDCD4呈低表达,二者异常表达均与年龄、病理分化分型、肿瘤直径、淋巴结转移相关,但对淋巴结转移的预测效能有限,还需结合其他指标综合评价。Objective To investigate the expression of SIRT3 and PDCD4 in skin squamous cell carcinoma and their relationship with clinicopathological features.Methods The clinical data(sex,age,pathological differentiation,tumor diameter,lymph node metastasis)of 209 patients with cutaneous squamous cell carcinoma were collected.The mRNA levels of SIRT3 and PDCD4 in the tissues were measured by qRT-PCR.The mRNA levels of SIRT3 and PDCD4 in the skin squamous cell carcinoma tissues were compared with those in the skin squamous cell carcinoma tissues,and the relationship between the expressions of SIRT3 and PDCD4 and the clinicopathological features of the skin squamous cell carcinoma was further analyzed.ROC curve was used to analyze the predictive value of SIRT3 and PDCD4 expression on lymph node metastasis in patients with skin squamous cell carcinoma.Results Compared with paracancer tissues,the SIRT3 level in squamous cell carcinoma tissues was significantly increased,and the PDCD4 expression level was significantly decreased(P<0.001).There were no statistical differences in the expression levels of SIRT3 and PDCD4 among patients with different genders(P>0.05),but there were statistical differences in the expression levels of SIRT3 and PDCD4 among patients with different ages,pathological differentiation types,tumor diameter and lymph node metastasis(P<0.05).In the prediction of lymph node metastasis,the prediction threshold of SIRT3 was 1.025,the sensitivity was 69.4%,and the specificity was 78.2%.The prediction threshold of PDCD4 was 0.478,and the sensitivity and specificity were poor.The AUC area of SIRT3,PDCD4 and their combination was 0.766,0.675 and 0.809,respectively.The AUC area of SIRT3,PDCD4 and their combination was the largest,and the sensitivity was higher than that of a single index,and the prediction efficiency was relatively good.Conclusion SIRT3 is highly expressed and PDCD4 is low expressed in squamous cell carcinoma of the skin.The abnormal expression of both is related to age,pathological differentiation,
