TLR4、GR、miRNA-15a对膀胱癌术后尿道感染的预测价值

摘　　要：目的探讨外周血Toll样受体4(TLR4)、微小RNA-15a(miRNA-15a)、糖皮质激素受体(GR)对膀胱癌术后尿道感染(UTI)的预测价值。方法选择行TURBT术治疗的90例膀胱癌患者,根据术后是否发生UTI分为2组,即感染组和未感染组,检测术后患者外周血中GR、TLR4、miRNA-15a水平。对比2组各项指标,分析影响UTI发生的危险因素,并绘制受试者工作特征(ROC)曲线,分析GR、miRNA-15a、TLR4预测TURBT术后患者发生UTI的价值。结果感染组TLR4检测值高于未感染组,GR、miRNA-15a检测值低于未感染组(P<0.05)。Logistic回归分析显示,年龄大、有糖尿病史、术前留置导尿管、miRNA-15a低、TLR4高、GR低是TURBT患者术后发生UTI的高危因素(P<0.05)。GR、miRNA-15a、TLR4联合检测预测术后UTI的AUC为0.934,灵敏度为0.900,特异度为0.817,约登指数为0.717,均高于单一检测。结论GR、TLR4、miRNA-15a与TURBT患者术后发生UTI密切相关,联合检测可提高预测价值。Objective To investigate the predictive value of toll-like receptor 4(TLR4),micrornA-15A(miRNA-15a)and glucocorticoid receptor(GR)in patients with urinary tract infection(UTI)after bladder cancer surgery.Methods 90 patients with bladder cancer who underwent TURBT surgery were divided into 2 groups according to whether UTI occurred after surgery,namely infected group and uninfected group.GR,TLR4 and miRNA-15a levels in peripheral blood of the patients were detected after surgery.The expression difference of various indicators between the 2 groups was compared,the risk factors affecting the occurrence of UTI were analyzed,and the receiver operating characteristic(ROC)curve was drawn to analyze the value of GR,miRNA-15a and TLR4 in predicting the occurrence of UTI in postoperative TURBT patients.Results The TLR4 values in the infected group were higher than those in the uninfected group,and the GR and miRNA-15a values were lower than those in the uninfected group(P<0.05).Logistic regression analysis showed that old age,history of diabetes,preoperative indignant catheter,low miRNA-15a,high TLR4 and low GR were risk factors for postoperative UTI in TURBT patients(P<0.05).The combined detection of GR,miRNA-15a and TLR4 predicted that the AUC of UTI was 0.934,the sensitivity was 0.900,the specificity was 0.817,and the Yoden index was 0.717,which were all higher than that of single detection.Conclusion GR,TLR4 and miRNA-15a are closely related to postoperative UTI in TURBT patients,and combined detection can improve the predictive value.
