术前FIB/CRP表达与进展期胃癌患者手术治疗预后的关系

摘　　要：目的探讨术前纤维蛋白原(FIB)/C反应蛋白(CRP)表达与进展期胃癌患者手术治疗预后的关系。方法回顾性收集接受胃癌根治术/姑息性手术治疗的110例进展期胃癌患者病历资料、术后随访资料,依据患者术后3年内生存情况,将31例病死患者归为预后不良组,其余79例患者归为预后良好组。比较两组基线资料和术前FIB、CRP等实验室指标,并计算FIB/CRP值。通过点二列相关性分析和受试者工作曲线(ROC)分析术前FIB/CRP表达与进展期胃癌患者手术治疗预后的关系。结果预后不良组Ⅳa期占比、CRP水平均显著高于预后良好组;FIB/CRP水平显著低于预后良好组,差异有统计学意义(P<0.05)。经点二列相关性分析结果显示,术前CRP、FIB/CRP与进展期胃癌患者不良预后有关(P<0.05)。绘制ROC曲线,结果显示,术前CRP、FIB/CRP预测进展期胃癌患者不良预后的曲线下面积(AUC)值分别为0.776、0.810,FIB/CRP预测价值最高。结论术前FIB/CRP与进展期胃癌患者手术治疗预后有关,其值越低,不良预后发生可能性越高,临床可通过术前FIB/CRP值来预测进展期胃癌患者手术治疗预后。Objective To explore the relationship between preoperative expression of fibrinogen(FIB)/C-reactive protein(CRP)and surgical treatment prognosis in patients with advanced gastric cancer.Methods Retrospective collected medical records and postoperative follow-up data from 110 patients with advanced gastric cancer who underwent surgical treatment.Based on the survival status of the patients within 3 years after surgery,31 deceased patients were classified as a poor prognosis group,while the remaining 79 patients were classified as a good prognosis group.Compare two sets of baseline data and preoperative laboratory indicators such as FIB and CRP,and calculate FIB/CRP values.Analyzing the relationship between preoperative FIB/CRP expression and surgical treatment prognosis in advanced gastric cancer patients through point two column correlation analysis and receiver operating curve(ROC)analysis.Results The proportion of stage IVa and CRP levels in the poor prognosis group were significantly higher than those in the good prognosis group.The FIB/CRP levels were significantly lower than those in the good prognosis group,with a statistical significant difference(P<0.05).The results of point 2 column correlation analysis showed that preoperative CRP,FIB/CRP were associated with poor prognosis in patients with advanced gastric cancer(P<0.05).ROC curve was drawn.The results showed that the area under the curve(AUC)values of preoperative CRP and FIB/CRP in predicting the poor prognosis of patients with advanced gastric cancer were 0.776 and 0.810,respectively.The higher the predictive value of FIB/CRP was.Conclusion Preoperative FIB/CRP is related to the surgical treatment prognosis of advanced gastric cancer patients.The lower the value,the higher the possibility of adverse prognosis.Clinical prediction of surgical treatment prognosis in advanced gastric cancer patients can be based on preoperative FIB/CRP values.
