摘　　要：目的分析四肢长骨骨巨细胞瘤(giant cell tumor of bone,GCTB)患者术后复发的相关危险因素。方法回顾性分析2012年5月至2022年5月云南省肿瘤医院(昆明医科大学第三附属医院)骨科收治的30例术后复发和40例术后未复发的四肢长骨GCTB患者的临床资料。采用多因素logistic回归分析分析影响术后复发的独立危险因素。结果70例患者均得到有效随访,随访时间为24~144个月,中位复发时间为21.4个月,发病高峰为20~29岁。患者术前Campanacci分级、是否短期应用地舒单抗或双膦酸盐辅助治疗均是术后复发的独立危险因素(均P<0.05)。结论四肢长骨GCTB的复发与Campanacci分级和是否行术前辅助治疗相关,而与术式的选择无关。Objective To analyze the related risk factors of the postoperative recurrence of giant cell tumor of bone(GCTB)in the long bones of extremities.Methods The clinical data of 30 patients with postoperative recurrence and 40 patients without postoperative recurrence of GCTB in the long bones of limbs admitted to the Department of Orthopaedics of Yunnan Cancer Hospital(the Third Affiliated Hospital of Kunming Medical University)from May 2012 to May 2022 were retrospectively analyzed.Multivariate logistic regression analysis was used to analyze the independent risk factors for postoperative recurrence.Results All 70 patients were effectively followed up,with a follow-up period of 24 to 144 months.The median recurrence time was 21.4 months,and the peak age of onset was 20 to 29 years old.Preoperative Campanacci grade and whether short-term denosumab or bisphosphonate adjuvant therapy was applied were independent risk factors for postoperative recurrence(all P<0.05).Conclusions The recurrence of GCTB in the long bones of limbs is related to Campanacci grade and whether preoperative adjuvant therapy is performed,and not related to the choice of surgical method.
