摘  要:目的:探究磁共振扩散加权成像(MRI-DWI)与超声检查在良恶性乳腺肿瘤鉴别中的诊断价值。方法:选取2021年1月至2023年12月甘肃省白银市第一人民医院影像中心60例乳腺病变患者,均接受MRI-DWI与超声检查,以组织病理学检查结果为金标准,统计MRI-DWI与超声单一检查及联合检查诊断结果,绘制受试者工作特征(ROC)曲线对比MRI-DWI与超声单一检查及联合检查的诊断效能。结果:60例乳腺病变患者中,经组织病理学检查确诊37例乳腺良性病变,占比61.67%;23例乳腺癌,占比38.33%;乳腺癌组ADC值显著低于良性乳腺病变组,P<0.05;超声检查联合MRI-DWI诊断准确度、灵敏度、特异度分别为93.33%、97.30%、86.96%,显著高于MRI-DWI(81.67%、91.89%、65.22%)及超声检查(70.00%、83.78%、47.83%),P<0.05;通过绘制ROC发现超声检查、MRI-DWI及联合诊断良恶性乳腺肿瘤的曲线下面积(AUC)值范围为0.658~0.921之间,其灵敏度和特异度较高,诊断良恶性乳腺肿瘤价值较高。结论:乳腺癌患者ADC值较乳腺良性病变显著较低,MRI-DWI联合超声检查在良恶性乳腺肿瘤鉴别诊断中具有较高的准确度和灵敏度,鉴别诊断价值较高。Objective:To investigate the diagnostic value of magnetic resonance diffusion-weighted imaging(MRI-DWI)and ultrasonography in the differentiation of benign and malignant breast tumours.Methods:Sixty patients with breast lesions were selected from the imaging centre of the first people's hospital of baiyin city,gansu province,from January 2021 to December 2023,all of them received MRI-DWI and ultrasound examination,and the diagnostic results of MRI-DWI and ultrasound single examination and combined examination were statistically based on the results of histopathological examination as the gold standard,and the working characteristics(ROC)curves of the subjects were drawn to compare the diagnostic efficacy of MRI-DWI and ultrasound single examination and combined examination.The ROC curves were plotted to compare the diagnostic efficacy of MRI-DWI with that of ultrasound.Results:Among 60 patients with breast lesions,37 cases of benign breast lesions were diagnosed by histopathological examination,accounting for 61.67%;23 cases of breast cancer,accounting for 38.33%;the ADC value of the breast cancer group was significantly lower than that of the benign breast lesion group,P<0.05;the diagnostic accuracy,sensitivity,and specificity of ultrasonography combined with MRI-DWI were 93.33%,97.30%,respectively,86.96%,which were significantly higher than those of MRI-DWI(81.67%,91.89%,65.22%)and ultrasonography(70.00%,83.78%,47.83%),P<0.05;by plotting R0C,it was found that the values of area under the curve(AUC)for the diagnosis of benign and malignant breast tumours by ultrasonography,MRI-DWI and the combined diagnosis ranged from 0.658~0.921,with high sensitivity and specificity and high value for diagnosing benign and malignant breast tumours.Conclusion:The ADC values of breast cancer patients were significantly lower than those of benign breast lesions,and MRI-DWI combined with ultrasonography had high accuracy and sensitivity in the differential diagnosis of benign and malignant breast tumours,with high differential dia
