CTP一站式检查联合CTA在急性缺血性脑卒中患者预后中的预测价值分析

摘　　要：目的:分析CT灌注成像(CTP)一站式检查联合CT血管造影(CTA)在急性缺血性脑卒中患者预后中的预测价值。方法:选取2022年5月至2022年11月期间某院收治的60例急性缺血性脑卒中患者作为研究对象,根据改良Rankin量表评分将其分为预后良好组与预后不良组,均进行CTP一站式检查与CTA检查,比较两组CTP参数、Alberta卒中项目早期CT评分(ASPECTS),绘制受试者工作特征(ROC)曲线对比诊断效能。结果:60例患者中预后良好组42例,预后不良组18例;预后良好组脑血容量(CBV)、脑血流量(CBF)水平高于预后不良组(P<0.05),平均通过时间(MTT)、达峰时间(TTP)水平低于预后不良组(P<0.05);预后良好组ASPECTS评分高于预后不良组(P<0.05);ASPECTS、CBV、CBF、MTT、TTP的曲线下面积(AUC)值在0.640~0.77之间,联合诊断AUC值高于单项指标诊断。结论:CTP一站式检查联合CTA可对急性缺血性脑卒中患者预后情况进行有效评估,具有较高预测价值。Objective:Analyze the predictive value of one-stop CT perfusion imaging(CTP)combined with CT angiography(CTA)in the prognosis of patients with acute ischemic stroke.Methods:Sixty patients with acute ischemic stroke admitted to a certain hospital between May 2022 and November 2022 were selected as the study subjects.They were divided into a group with good prognosis and a group with poor prognosis according to the classification criteria in the improved Rankin scale.CTP one-stop examination and CTA examination were conducted for both groups.CTP parameters and Alberta Stroke Early CT Score(ASPECTS)were compared between the two groups,and receiver operating characteristic(ROC)curves were drawn to compare diagnostic efficacy.Results:Among the 60 patients,there were 42 cases in the group with good prognosis and 18 cases in the group with poor prognosis;The levels of cerebral blood volume(CBV)and cerebral blood flow(CBF)in the group with good prognosis were higher than those in the group with poor prognosis(P<0.05),while the average transit time(MTT)and peak time(TTP)were lower than those in the group with poor prognosis(P<0.05);The ASPECTS score of the group with good prognosis was higher than that of the group with poor prognosis(P<0.05);The area under the curve(AUC)values of ASPECTS,CBV,CBF,MTT,and TTP are between 0.640 and 0.77,and the combined diagnosis has a higher AUC value than the single indicator diagnosis.Conclusion:The combination of CTP one-stop examination and CTA can effectively predict the prognosis of patients with acute ischemic stroke,and has high diagnostic efficacy.
