摘 要:目的:探究磁共振弥散加权成像(DWI)与常规磁共振成像(MRI)在短暂性脑缺血发作(TIA)诊断中的应用。方法:回顾性分析我院于2020年6月至2022年12月收治的60例TIA患者临床资料,所有患者均行常规MRI及DWI检查,记录检查序列检出率,并采用Logistic回归分析DWI、常规MRI检出TIA阳性的影响因素。结果:60例患者中采用DWI进行检测,存在典型点状异常高信号影患者共24例,其中单一责任病灶19例,多发责任病灶5例,阳性检出率为40.00%;MRI中T2WI存在点状异常高信号影患者5例,阳性检出率为13.33%,DWI阳性检出率高于MRI(P<0.05);DWI阳性组与DWI阴性组患者临床资料比较,年龄、发作时间、症状(失语、运动障碍)差异有统计学意义(P<0.05);MRI阳性组与MRI阴性组患者临床资料比较,发作时间、症状(失语、运动障碍)差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示:年龄、发作时间、失语、运动障碍为TIA患者DWI阳性的影响因素(P<0.05);发作时间、失语、运动障碍为TIA患者MRI阳性的影响因素(P<0.05)。结论:在TIA诊断中,DWI较常规MRI诊断价值高,发作时间、失语、运动障碍均为DWI与常规MRI检出阳性的影响因素,临床应对上述影响因素引起重视,及时采取措施进行干预。Objective:To explore the application of diffusion-weighted magnetic resonance imaging(DWI)and conventional magnetic resonance imaging(MRI)in the diagnosis of transient ischemic attack(TIA).Methods:The clinical data of 60 TIA patients admitted to our hospital from June 2020 to December 2022 were retrospectively analyzed.All patients underwent conventional MRI and DWI examinations.The detection rates of examination sequences were recorded,and logistic regression was used to analyze the influencing factors of positive detection of TIA by DWI and conventional MRI.Results:Among the 60 patients detected by DWI,there were a total of 24 patients with typical punctate abnormal high signal shadows,including 19 cases of single responsible lesions and 5 cases of multiple responsible lesions.The positive detection rate was 40.00%.In MRI,there were 5 patients with punctate abnormal high signal shadows in T2WI,and the positive detection rate was 13.33%.The positive detection rate of DWI was higher than that of MRI(P<0.05).Comparing the clinical data of patients in the DWI positive group and the DWI negative group,there were statistically significant differences in age,onset time,and symptoms(aphasia,dyskinesia)(P<0.05).Comparing the clinical data of patients in the MRI positive group and the MRI negative group,there were statistically significant differences in onset time and symptoms(aphasia,dyskinesia)(P<0.05).The results of multivariate logistic regression analysis showed that age,onset time, aphasia, and dyskinesia were the influencing factors for positive DWI in TIA patients (P< 0. 05). Onset time,aphasia, and dyskinesia were the influencing factors for positive MRI in TIA patients (P<0. 05). Conclusion: In thediagnosis of TIA, DWI has higher diagnostic value than conventional MRI. Onset time, aphasia, and dyskinesia are allinfluencing factors for positive detection by DWI and conventional MRI. Clinically, attention should be paid to the aboveinfluencing factors and timely measures should be taken for intervention.
