冠脉CTA和DSA在诊断心肌桥中的价值探讨

摘　　要：目的:评估冠脉计算机断层血管摄影(CTA)、数字减影血管造影(DSA)应用在心肌桥(MB)诊断中的价值。方法:纳入2022年1月至2023年1月的150例MB患者,均行冠脉CTA、DSA,评价二者诊断准确率、心肌桥数分布情况。结果:(1)较之DSA,冠脉CTA的漏诊率、误诊率更低,并且诊断准确率更高,P<0.05;(2)金标准结果显示MB在左冠回旋支46例、左冠前降支54例、右冠脉22例、对角支28例,冠脉CTA诊断准确率为100.00%、100.00%、90.91%、92.86%,均高于DSA的76.09%、70.37%、54.55%、64.29%,P<0.05。结论:在MB诊断中采用冠脉CTA,诊断准确率高,并且可以清晰显示MB分布特点,值得临床推广。Objective:To evaluate the value of coronary computed tomography angiography(CTA)and digital subtraction angiography(DSA)in the diagnosis of myocardial bridge(MB).Methods:150 MB patients from January 2022 to January 2023 were included.All patients underwent coronary CTA and DSA to evaluate the diagnostic accuracy and the distribution of myocardial bridge numbers of the two methods.Results:(1)Compared with DSA,coronary CTA had lower missed diagnosis rate and misdiagnosis rate and higher diagnostic accuracy,P<0.05;(2)The gold standard results showed that there were 46 cases of MB in the left circumflex coronary artery,54 cases in the left anterior descending coronary artery,22 cases in the right coronary artery,and 28 cases in the diagonal branch.The diagnostic accuracy rates of coronary CTA were 100.00%,100.00%,90.91%,and 92.86%,respectively,which were all higher than those of DSA(76.09%,70.37%,54.55%,and 64.29%),P<0.05.Conclusion:The use of coronary CTA in the diagnosis of MB has high diagnostic accuracy and can clearly show the distribution characteristics of MB,which is worthy of clinical promotion.
