摘　　要：目的:探讨残胃癌(GSC)的临床及MSCT特征。方法:回顾性分析本院经病理证实17例GSC患者的临床及CT特征。结果:本组共17例GSC患者,男性14例,女性3例;病理证实腺癌14例(82.4%)。发生于吻合口10例(58.8%),非吻合口7例(41.2%);发生于吻合口CT表现为吻合口局部增厚;发生于残胃壁早期表现为黏膜增厚,进展期胃壁弥漫性不均匀增厚、僵硬,边缘凹凸不平,浆膜面模糊。平扫呈等密度;动脉期、静脉期增厚的黏膜或胃壁明显强化,黏膜层连续性中断;延迟期呈等密度。易伴胃周及腹主动脉旁淋巴结转移。结论:GSC临床症状及体征不典型,肿瘤标志物无特异性;MSCT可清晰显示病灶的位置、强化特征及转移情况,对临床治疗方案的选择具有重要意义。Objective:To explore the clinical features and MSCT findings of GSC.Methods:The clinical and imaging data of patients with GSC confirmed in our hospital from January 1,2015 to September 30,2022 were retrospectively collected and analyzed.Results:There were 17 patients with GSC,included 14 males and 3 females;14 cases(82.4%)of adenocarcinoma proved by pathology.10 cases(58.8%)occurred at anastomotic site and 7 cases(41.2%)at nonanastomotic site;CT showed local thickened anastomotic site and remnant gastric wall in the early stage,Diffuse uneven thickening and stiffness of gastric wall in the advanced stage,the edge was uneven and the serous surface of the stomach was vague,CT scanning showed iso-density,the thickened mucosa or gastric wall was significantly enhanced in arteriovenous phase and showed high-density,the continuity of mucosal layer was interrupted,and delayed phase showed iso-density;It was easily accompanied by lymph node metastasis around stomach and abdominal aorta.Conclusion:The clinical symptoms and signs of GSC were not typical,and the tumor markers had no specificity;MSCT could clearly show the location,enhancement characteristics and metastasis of the lesions,which is of great significance for the choice of clinical treatment.
