摘 要:目的对比分析超声引导微波消融术与传统手术治疗乳腺良性结节的效果。方法队列研究。抽取2019年1月至2023年1月南阳医学高等专科学校第一附属医院收治的乳腺良性结节患者68例, 按随机数字表法分为微创组和传统组, 每组34例。传统组采取传统手术治疗, 微创组采取超声引导微波消融术治疗。对比两组临床疗效、围术期情况、炎症因子水平、数字评分法(NRS)评分及并发症发生率。结果微创组总有效率(91.18%, 31/34)与传统组(94.12%, 32/34)比较差异未见统计学意义(P>0.05)。两组手术时间比较差异未见统计学意义(P>0.05);微创组术中出血量少于传统组, 切口愈合时间短于传统组(P<0.05)。术后3 d, 微创组肿瘤坏死因子、白细胞介素-6、超敏C反应蛋白水平低于传统组(P<0.05)。术后1、3 d, 微创组NRS评分低于传统组(P<0.05)。微创组并发症发生率(2.94%, 1/34)低于传统组(29.41%, 10/34), P<0.05。结论和传统手术相比, 超声引导微波消融术治疗乳腺良性结节患者, 可减少术中出血量, 缩短切口愈合时间, 减轻炎症反应, 减轻疼痛, 降低并发症发生率。Objective To compare the effect of ultrasound-guided microwave ablation and conventional surgery in the treatment of benign breast nodules.MethodsSixty-eight patients with benign breast nodules treated in the First Affiliated Hospital of Nanyang Medical College from January 2019 to January 2023 were selected,and they were divided into the minimally invasive group and the traditional group using a random number table method,with 34 cases in each group.The traditional group was treated by traditional surgery,and the minimally invasive group was treated by ultrasound-guided MWA.The clinical efficacy,perioperative conditions,levels of inflammatory factors,numerical rating scale(NRS)score and complication rate were compared between the two groups.ResultsThere was no significant difference in the total effective rate between the minimally invasive group(91.18%,31/34)and the traditional group(94.12%,32/34),P>0.05.There was no significant difference in operation duration between the two groups(P>0.05).The amount of bleeding in the minimally invasive group was less than that in the traditional group,and the healing time of incision was shorter than that in the traditional group(P<0.05).Three days after operation,the levels of tumor necrosis factor-α,interleukin-6 and high-sensitivity C-reactive protein in the minimally invasive group were lower than those in the traditional group(P<0.05).On the 1st and 3rd days after operation,the NRS scores in the minimally invasive group were lower than those in the traditional group(P<0.05).The incidence of complications in the minimally invasive group(2.94%,1/34)was lower than that in the traditional group(29.41%,10/34),P<0.05.ConclusionsCompared with traditional surgery,ultrasound-guided microwave ablation can reduce intraoperative blood loss,shorten incision healing time,alleviate inflammation,relieve pain,and reduce the incidence of complications.
