摘　　要：目的探讨钼靶、选择性乳腺导管造影、高频彩色多普勒超声对良恶性乳头溢液疾病的鉴别诊断价值。方法回顾性抽取2021年6月至2022年11月洛阳市妇幼保健院收治的乳头溢液患者82例, 统计钼靶、选择性乳腺导管造影、高频彩色多普勒超声、选择性乳腺导管造影联合高频彩色多普勒超声对乳头溢液性质的诊断效能。结果 82例乳头溢液患者中, 手术病理确诊恶性37例, 良性45例;乳腺钼靶诊断恶性38例, 良性44例;选择性乳腺导管造影诊断恶性41例, 良性41例;高频彩色多普勒超声诊断恶性39例, 良性43例。选择性乳腺导管造影和高频彩色多普勒超声单独及联合诊断乳头溢液性质的敏感性、特异性及准确度高于乳腺钼靶, 高频彩色多普勒超声诊断准确度高于选择性乳腺导管造影, 选择性乳腺导管造影联合高频彩色多普勒超声诊断敏感性、特异性及准确度高于单独诊断。结论高频彩色多普勒超声联合选择性乳腺导管造影在乳头溢液良恶性鉴别诊断中具有较高的价值。Objective To investigate the value of molybdenum,selective ductography and high-frequency color Doppler ultrasound in the differential diagnosis of benign and malignant nipple discharge diseases.MethodsEighty-two patients with nipple discharge admitted to Luoyang Maternal and Child Health Care Hospital from June 2021 to November 2022 were retrospectively selected,and the efficacy of molybdenum,selective ductography,high-frequency color Doppler ultrasound,and selective ductography combined with high-frequency color Doppler ultrasound in the diagnosis of nature of nipple discharge was counted.ResultsAmong the 82 patients with nipple discharge,37 cases were malignant,and 45 cases were benign according to pathological examination.Molybdenum identified 38 malignant cases and 44 benign cases.Selective ductography identified 41 malignant cases and 41 benign cases.High-frequency color Doppler ultrasound identified 39 malignant cases and 43 benign cases.The sensitivity,specificity and accuracy of selective ductography alone,high-frequency color Doppler ultrasound alone,and combination of them in diagnosing the nature of nipple discharge were higher than those of ductography;the diagnostic accuracy of high-frequency color Doppler ultrasound was higher than that of selective ductography;and the sensitivity,specificity and accuracy of selective ductography combined with high-frequency color Doppler ultrasound were higher than those of the two single examination methods.ConclusionsHigh-frequency color Doppler ultrasound combined with selective ductography has high value in the differential diagnosis of benign and malignant nipple discharge.
