摘 要:目的探讨血清白细胞介素(IL)-17、IL-35水平与急性淋巴细胞白血病(ALL)患儿危险度分层和预后的关系。方法根据危险度分层标准将100例ALL患儿分为标危组(26例)、中危组(51例)、高危组(23例)。比较3组患儿IL-17、IL-35水平,并分析IL-17、IL-35水平与危险度分层的相关性。对所有患儿随访1年,比较预后良好与预后不良患儿的临床特征。采用Logistic回归模型分析ALL患儿预后的影响因素。结果中危组、高危组患儿IL-17水平均低于标危组,IL-35水平均高于标危组,高危组患儿IL-17水平低于中危组,IL-35水平高于中危组,差异均有统计学意义(P﹤0.05)。IL-17水平与ALL患儿危险度分层呈负相关(r=-0.322,P=0.023),IL-35水平与危险度分层呈正相关(r=0.284,P=0.034)。随访1年,预后良好患儿62例,预后不良患儿38例,预后良好与预后不良患儿白细胞计数、IL-17水平、IL-35水平比较,差异均有统计学意义(P﹤0.05)。多因素Logistic回归分析结果显示,IL-17、IL-35水平均为ALL患儿预后的影响因素(P﹤0.05)。结论IL-17水平与ALL患儿的危险度分层呈负相关,IL-35水平与危险度分层呈正相关。IL-17、IL-35水平均为ALL患儿预后的影响因素,IL-17水平越低,IL-35水平越高,患儿预后越差。Objective To explore the relationship between the levels of serum interleukin(IL)-17,IL 35 and risk stratification and prognosis of children with acute lymphoblastic leukemia(ALL).Method According to the risk stratifi-cation criteria,100 children with ALL were divided into standard risk group(26 cases),medium risk group(51 cases)and high risk group(23 cases).The levels of IL-17 and IL-35 in three groups were compared,and the correlation between the levels of IL-17,IL-35 and risk stratification were analyzed.All children were followed up for 1 year,and the clinical char-acteristics of the children with good prognosis and poor prognosis were compared.Logistic regression model was used to analyze the prognostic factors of children with ALL.Result The levels of IL-17 in medium risk group and high risk group were lower than those in standard risk group,the levels of IL-35 were higher than those in standard risk group,the level of IL-17 in high risk group was lower than that in medium risk group,the level of IL-35 was higher than that in me-dium risk group,and the differences were statistically significant(P<0.05).The level of IL-17 was negatively correlated with risk stratification of ALL children(r=-0.322,P=0.023),the level of IL-35 was positively correlated with risk stratifi-cation(r=0.284,P=0.034).After one year follow-up,there were 62 children with good prognosis and 38 children with poor prognosis.There were statistically significant differences in white blood cell count,IL-17 level and IL-35 level be-tween the children with good prognosis and poor prognosis(P<0.05).Multivariate Logistic regression analysis showed that the levels of IL-17 and IL-35 were influencing factors for the prognosis of children with ALL(P<0.05).Conclusion The level of IL-17 is negatively correlated with risk stratification in children with ALL,while IL-35 level is positively correlated with risk stratification.The levels of IL-17 and IL-35 are influencing factors for the prognosis of children with ALL.The lower level of IL-17,the higher
