摘　　要：目的探讨人工股骨头置换术(AFHR)治疗股骨转移瘤患者的疗效。方法根据治疗方式的不同将100例股骨转移瘤患者分为观察组(n=49,AFHR治疗)和对照组(n=51,常规治疗)。比较两组患者的治疗效果、术后疼痛情况[视觉模拟评分法(VAS)]、平衡能力[Berg平衡量表(BBS)]、下肢运动功能[Fugl-Meyer运动功能评估量表(FMA)]及并发症发生情况。结果观察组患者总有效率为95.92%,高于对照组患者的80.39%,差异有统计学意义(P﹤0.05)。术后1、2、3周,观察组患者VAS评分均明显低于对照组,差异均有统计学意义(P﹤0.01)。术后3周,两组患者BBS、FMA评分均高于本组术前,观察组患者BBS、FMA评分均高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者的并发症总发生率为6.12%,与对照组患者的13.73%比较,差异无统计学意义(P﹤0.05)。结论AFHR治疗股骨转移瘤患者疗效显著,可减轻患者术后疼痛,改善平衡能力和下肢功能,并且不会增加并发症。Objective To investigate the efficacy of artificial femoral head replacement(AFHR)in the treatment of patients with femoral metastases.Method A total of 100 patients with femoral metastases were divided into observation group(n=49,AFHR treatment)and control group(n=51,conventional treatment)according to different treatment meth-ods.The treatment effect,postoperative pain[visual analogue scale(VAS)],balance ability[Berg balance scale(BBS)],lower limb motor function[Fugl-Meyer assessment(FMA)]and the occurrence of complications were compared between the two groups.Result The total effective rate of observation group was 95.92%,which was higher than 80.39%of con-trol group,and the difference was statistically significant(P<0.05).At 1,2 and 3 weeks after operation,the scores of VAS in observation group were significantly lower than those in control group,and the differences were statistically significant(P<0.01).At 3 weeks after surgery,the scores of BBS and FMA in two groups were higher than those before surgery,the scores of BBS and FMA in observation group were higher than those in control group,and the differences were statistical-ly significant(P<0.05).The total incidence of complications in the observation group was 6.12%,and there was no signif-icant difference compared with 13.73%in the control group(P<0.05).Conclusion AFHR is effective in the treatment of patients with femoral metastases.It can reduce postoperative pain,improve balance ability and lower limb function,and does not increase complications.
