活血柔肝养阴方联合督脉十三针治疗脑梗死后下肢痉挛的临床研究

摘　　要：目的:探讨活血柔肝养阴方联合督脉十三针治疗脑梗死后下肢痉挛患者的临床疗效。方法:选取2022年1月~2023年12月期间我院收治的66例脑梗死后下肢痉挛患者作为研究对象,采用随机数字表法分为对照组和观察组,每组33例。两组患者均接受常规干预,对照组患者在常规干预基础上接受督脉十三针治疗,观察组患者在对照组治疗基础上给予活血柔肝养阴方治疗,两组均持续治疗4周。比较两组患者中医证候积分、下肢痉挛程度[综合痉挛量表(CSS)评分]、平衡功能[伯格氏平衡量表(BBS)评分]、临床疗效及不良反应发生情况。结果:治疗后,两组患者主要和次要证候积分均降低(P<0.05),且观察组低于对照组(P<0.05);CSS评分均降低(P<0.05),且观察组低于对照组(P<0.05);BBS评分均升高(P<0.05),且观察组高于对照组(P<0.05);观察组患者治疗总有效率(96.97%)高于对照组(75.76%,P<0.05)。两组患者不良反应总发生率比较无统计学差异(P>0.05)。结论:活血柔肝养阴方联合督脉十三针可显著改善患者临床症状,缓解下肢痉挛程度,提高肢体平衡能力,且不会增加不良反应。Objective:To investigate the clinical efficacy of Huoxue Rougan Yangyin decoction combined with Du Mai thirteen acupuncture in the treatment of lower limb spasms following cerebral infarction.Methods:A total of 66 patients experiencing lower limb spasms following cerebral infarction admitted to a hospital from January 2022 to December 2023 were selected and divided into the control group and the observation group by random number table method,with 33 patients in each group.Patients in both groups received routine interventions.The control group underwent Du Mai thirteen acupuncture treatment in addition to the routine interventions,while the observation group was given Huoxue Rougan Yangyin decoction alongside the control group treatment.Both groups continued treatment for 4 weeks.The traditional Chinese medicine syndrome score,the degree of lower limb spasms[composite spasticity scale(CSS)score],balance function[Berg balance scale(BBS)score],clinical efficacy and adverse reactions were compared between the two groups.Results:Following treatment,the scores for both major and minor syndroms were decreased in both groups(P<0.05),with the observation group showing lower scores than the control group(P<0.05).The CSS scores was decreased in both groups(P<0.05),with the observation group showing lower scores than the control group(P<0.05).The BBS scores was increased in both groups(P<0.05),with the observation group exhibiting higher scores than the control group(P<0.05).The total effective rate of observation group(96.97%)was higher than that of control group(75.76%,P<0.05).No statistically significant difference was observed in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of Huoxue Rougan Yangyin decoction and Du Mai thirteen acupuncture significantly improves the clinical symptoms of patients,alleviates the degree of lower limb spasms,enhances the ability of limb balance,and does not increase the incidence of adverse reactions.
