慢性硬膜下血肿患者钻孔引流术后使用桃红四物汤加减联合常规西药治疗的效果分析

摘　　要：目的:评估慢性硬膜下血肿患者钻孔引流术后使用桃红四物汤加减联合常规西药治疗的效果。方法:选择2021年1月-2023年1月于郓城县中医医院行钻孔引流术治疗的慢性硬膜下血肿患者60例作为研究对象,采用随机数字表法分为常规组、联合组,每组30例。常规组术后使用常规西药治疗,联合组在常规组基础上使用桃红四物汤加减治疗。对比两组治疗效果、神经功能、血肿体积。结果:联合组治疗总有效率为96.67%,高于常规组(P<0.05)。术前,两组中国脑卒中临床神经功能缺损程度评分量表(CSS)评分比较,差异无统计学意义(P>0.05);术后用药4周,两组CSS评分下降,且联合组低于常规组(P<0.05)。术前,两组血肿体积比较,差异无统计学意义(P>0.05);术后用药4周,两组血肿体积缩小,且联合组小于常规组(P<0.05)。结论:慢性硬膜下血肿患者钻孔引流术后使用桃红四物汤加减联合常规西药治疗的效果显著,能够改善神经功能,促进血肿体积缩小。Objective:To evaluate the effect of modified Taohong Siwu Decoction combined with western medicine treatment in patients with chronic subdural hematoma after trepanation and drainage surgery.Methods:A total of 60 patients with chronic subdural hematoma who underwent trepanation and drainage surgery in Yuncheng Hospital of Traditional Chinese Medicine from January 2021 to January 2023 were selected as the study subjects.They were divided into conventional group and combination group according to random number table method,with 30 patients in each group.Conventional group received postoperative conventional western medicine treatment,and the combination group was treated with modified Taohong Siwu Decoction on the basis of conventional group.The treatment effect,neurological function and hematoma volume were compared between the two groups.Results:The total effective rate of treatment in the combination group was 96.67%,which was higher than that in the conventional group(P<0.05).There was no significant difference in the scores of the Chinese Stroke Scale for Clinical Neurological Deficit(CSS)between the two groups(P>0.05).After 4 weeks of postoperative drug treatment,the CSS scores in the two groups decreased and the scores in the combination group were lower than those in the conventional group(P<0.05).There was no statistically significant difference in hematoma volume between the two groups before operation(P>0.05).After 4 weeks of postoperative drug treatment,the hematoma volume in the two groups decreased and the volume in the combination group was smaller than that in the conventional group(P<0.05).Conclusion:The modified Taohong Siwu Decoction combined with western medicine treatment in patients with chronic subdural hematoma after trepanation and drainage surgery has significant effects,and can improve neurological function and promote the reduction of hematoma volume.
