摘  要:目的:应用锥形束CT(CBCT)观察上颌第二前磨牙根管形态及其增龄性改变,为临床治疗提供参考依据。方法:选取343例患者的600个上颌第二前磨牙的锥形束CT影像资料,按年龄分为6组:≤20岁组、21~30岁组、31~40岁组、41~50岁组、51~60岁组及>60岁组。观察上颌第二前磨牙根管数目和根管类型,测量颊侧髓角高度、舌侧髓角高度、颊舌侧髓角间距、根管口间距,并分析以上指标与年龄的关系。结果:上颌第二前磨牙单根管的发生率为65.67%,双根管的发生率为34.33%。各年龄组间双根管的发生率比较,差异具有统计学意义(P<0.05),双根管的发生率随着年龄的增加有不断增高的趋势(P<0.05)。上颌第二前磨牙根管类型以Ⅰ型单根管为主,发生率为66%;双根管系统中,Ⅲ型最多见13.83%,其次是Ⅱ型7%,Ⅴ型6.67%,Ⅳ型3.33%,其他型1.83%,Ⅵ型和Ⅶ型发生率均为0.83%。各年龄组间上颌第二前磨牙颊侧髓角高度、舌侧髓角高度、颊舌侧髓角间距、根管口间距比较,差异均具有统计学意义(P<0.05),且随着年龄的增加有不断减少的趋势(P<0.05)。结论:上颌第二前磨牙根管系统解剖形态具有增龄性改变,随着年龄的增加,根管形态结构更加复杂,锥形束CT影像可为临床根管治疗提供可靠的依据。AIM:The root canal morphology of maxillary second premolars and the changes with aging were observed by using cone beam computed tomography(CBCT)to provide reference for clinical treatments.METHODS:A total of 600 CBCT images of maxillary second premolars were collected from 343 patients retrospectively.The subjects were divided into 6 groups according to the patients’ages:groups≤20,21~30,31~40,41~50,51~60 and>60 years old,respectively.The number and type of root canals of maxillary second premolars were observed.The height of buccal and lingual medullary angles,the distance between buccal and lingual medullary angles and the diameter of root canal orifices were measured,and their correlation with age were analyzed.RESULTS:The incidence of single root canal and double root canal in maxillary second premolars were 65.67%and 34.33% respectively.There were statistical significance in the incidence of double root canals among different age groups(P<0.05),and the incidence of double root canals gradually increased with aging(P<0.05).The root canal type of maxillary second premolars were mainly type Ⅰ 66%.In the double root canals system,the incidence of type Ⅲ was 13.83%,followed by the type Ⅱ 7%,type V 6.67%,type Ⅳ 3.33%,other type 1.83%,type Ⅵ 0.83% and type Ⅶ 0.83%,respectively.The height of buccal and lingual medullary angles,the distance between buccal and lingual medullary angles and the diameter of root canal orifices of maxillary second premolars were statistically significant among different age groups(P<0.05),and they gradually decreased with aging(P<0.05).CONCLUSION:The anatomical morphology of the root canal system of maxillary second premolars changes with aging.The morphology root canal becomes more complex with age increasing.The findings of cone beam CT images may provide a reliable evidence for clinical treatment of root canal.
