综合性干预手段对伴有偏侧咀嚼的无牙颌患者全口义齿修复后的疗效评估

摘　　要：目的:比较伴有偏侧咀嚼的无牙颌患者经过全口义齿修复后通过综合性干预手段前后的咀嚼效率、咀嚼肌的不对称指数等的变化,评估干预后的治疗效果。方法:选取2021年10月~2023年6月期间我院口腔修复科就诊,通过检查筛选出既往存在偏侧咀嚼超过6个月的无牙颌患者30名,年龄(69.2±2.97)岁。按随机数字表法随机分为两组,每组15名,干预组为全口义齿修复后综合性干预手段为主,对照组为全口义齿修复后不做任何处理。两组分别用ViewGum色彩分析软件和肌电图仪(electromyography,EMG)测试修复后即刻,3、6个月的废用侧咀嚼效率以及咀嚼运动时废用侧和惯用侧的咬肌(左:LMM,右:RMM)和颞肌前束(左:LTA,右:RTA)的不对称指数。结果:全口义齿修复后即刻咀嚼效率差异没有统计学意义(P>0.05);综合性干预治疗后3、6个月各时间点,干预组废用侧色调值标准差(SDHue)明显小于对照组废用侧SDHue,即综合性干预治疗后3、6个月各时间点,干预组废用侧咀嚼效率明显优于对照组,差异有统计学意义(P<0.05)。全口义齿修复后3、6个月干预组咀嚼肌的不对称指数低于对照组,但差异无统计学意义(P>0.05)。结论:伴有偏侧咀嚼的无牙颌患者全口义齿修复后行综合性干预治疗后的咀嚼效率明显改善,但咀嚼肌的不对称指数没有明显变化。Objective:To compare the changes of chewing efficiency and asymmetry index of masticatory muscles in edentulous patients with lateral chewing before and after full denture restoration by comprehensive intervention,and to evaluate the treatment effect after intervention.Methods:From October 2021 to June 2023,30 edentulous patients with hemichewing for more than 6 months were selected through examination,aged(69.2±2.97)years.The intervention group with comprehensive intervention methods after full denture restoration,and the control group without any treatment after full denture restoration.ViewGum color analysis software and electromyography(EMG)were used to test the masticatory efficiency of the discarded side and the asymmetry index of the masseter muscle(left:LMM,right:RMM)and the anterior temporalis tract(left:LTA,right:RTA)on the discarded and dominant sides during masticatory exercise at 0,3,and 6 months after repair,respectively.Results:There was no significant difference in mastication efficiency at 0 months(P>0.05).At each time point of 3 and 6 months after comprehensive intervention treatment,SDHue value of the disused side in the intervention group was significantly lower than that in the control group,indicating that the masticatory efficiency of the disused side in the intervention group was significantly better than that in the control group at each time point of 3 and 6 months after comprehensive intervention treatment,and the difference was statistically significant(P<0.05).The asymmetry index of masticatory muscle in intervention group at 3 and 6 months was lower than that in control group,but the difference was not statistically significant(P>0.05).Conclusion:The chewing efficiency of edentulous patients with hemichewing is significantly improved after comprehensive intervention after full denture restoration,but the asymmetry index of masticatory muscle is not significantly changed.
