从“天一生水”论扶阳四逆法的立法之意和运用思路

摘  要:扶阳学派是川派医学中最具特色的学术流派,虽显年轻却传播影响广大。四逆法是扶阳医学立法处方的一大法门,通过合理配伍能以一法衍百方。文章基于“天一生水”的数与理,论述其内蕴的重阳、藏精、崇土思想,进而探析四逆法的立法之意,将临床四逆法的主要运用变化归纳为四逆合开宣、温通、温补填精三法,三法相互联系,呈现一定的运用次第。四逆法的运用具备一定的禁忌,在实热证、表证未解、里气不通的情况下当忌用或慎用。对四逆法立意和运用的探究与归纳有助于深化理解扶阳医学的理论内涵并启发临床思路。Fuyang school is the most distinctive academic school within Sichuan medicine,young yet widely influential.The Sini method is a fundamental approach in Fuyang medicine for formulating prescriptions,capable of deriving numerous recipes through rational combinations.Based on the numerical and philosophical concept of‘Tian Yi Sheng Shui’,this paper elaborates on the underlying ideas of emphasizing yang,storing essence,and venerating earth within the Fuyang school.It further explores the legislative intent behind the Sini method and summarizes its main clinical applications into three variations:Sini combined with opening and dispersing,warming and unblocking,and warming,tonifying,and replenishing essence.These three methods are interrelated and exhibit a certain sequence of application.The use of the Sini method carries certain contraindications and should be avoided or used with caution in cases of excess heat,unresolved exterior syndromes,and interior qi obstruction.The exploration and summarization of the conceptual framework and application of the Sini method contribute to a deeper understanding of the theoretical connotations of Fuyang medicine and inspire clinical thinking.
