摘 要:目的探究食管鳞状细胞癌(ESCC)中Kin17的表达,分析其与不同临床病理参数的相关性及预后价值。方法应用UALCAN在线分析工具比较癌症基因组图谱(TCGA)数据库中Kin17在食管癌组织和正常组织中mRNA表达水平差异,同时对GSE23400中53对来自ESCC和相邻正常组织的Kin17基因表达进行比较。收集2016年7月-2017年6月在广州医科大学附属肿瘤医院手术切除并经病理确诊为ESCC的63例患者的石蜡组织切片及临床病理资料,包括肿瘤组织和正常组织。采用免疫组织化学检测上述患者组织中Kin17蛋白的表达,计算免疫组化积分并进行组间比较。分析ESCC患者与各项临床病理特征在Kin17不同表达水平之间的差异性,并进一步分析Kin17不同表达水平对ESCC患者生存结局的影响。结果TCGA、GSE23400数据集中,Kin17 mRNA在肿瘤组织中的表达显著高于相应的正常组织,差异均有统计学意义(P均<0.001)。免疫组化染色显示,Kin17蛋白的强阳性染色主要存在于ESCC肿瘤细胞的细胞核中,正常组织仅有部分有较弱强度的着色;Kin17蛋白在肿瘤组织表达同样显著高于正常组织,差异有统计学意义(P<0.001)。临床病理参数中仅肿瘤细胞分化程度、Ki⁃67阳性率、淋巴结转移及pTNM分期在Kin17不同表达水平间差异均有统计学意义(χ^(2)=7.800、5.763、8.453、9.938,P均<0.05)。受试者工作特征曲线显示,Kin17预测ESCC患者无进展生存期和总生存期曲线下面积分别为0.656和0.707。K⁃M生存曲线表明,高Kin17表达与较差的无进展生存时间(P=0.030)和总生存时间(P=0.003)生存结局相关。结论Kin17在ESCC患者中高表达,对应的ESCC患者的肿瘤细胞级别更高、淋巴结有转移、pTNM分期更晚、Ki⁃67阳性率更高。Kin17表达水平对ESCC的生存结局有一定预测价值,高水平表达会导致ESCC患者的不良预后。Objective To explore the expression of Kin17 in esophageal squamous cell carcinoma(ESCC)and analyze its correlation with different clinicopathological parameters and its prognostic value.Methods The UALCAN online analysis tool was used to compare the differences in the mRNA expression levels of Kin17 in esophageal cancer in the The Cancer Genome Atlas(TCGA)database,and the expression levels of Kin17 in 53 pairs of ESCC and adjacent normal tissues in GSE23400 were compared.The paraffin tissue sections and clinical pathological data of 63 patients who underwent surgical resection and pathologically confirmed ESCC in the the Guangzhou Institute of Cancer Research,the Affiliated Cancer Hospital,Guangzhou Medical University from July 2016 to June 2017 were collected,including tumor tissue and paracancerous tissue.Immunohistochemistry was used to detect the expression of Kin17 protein in the tissues of the above patients,and the immunohistochemical scores were calculated and compared between groups.The correlation between Kin17 expression and various clinicopathological characteristics,and the impact of different expression levels of Kin17 on the survival outcomes of ESCC patients were analyzed.Results In the TCGA and GSE23400 databases analysis showed the expression of Kin17 mRNA in tumor tissues was significantly higher than that in corresponding normal tissues(all P<0.001).Immunohistochemistry staining showed that the strong positive staining of Kin17 protein mainly existed in the nuclei of ESCC tumor cells,and only part of the normal tissue adjacent to the cancer had weaker intensity staining;the expression of Kin17 protein in the tumor tissue was also significantly higher than that of the adjacent normal tissue(P<0.001).Among the clinicopathological parameters,only the degree of tumor cell differentiation,Ki⁃67 positivity rate,lymph node metastasis and pTNM stage had statistically significant differences at different expression levels of Kin17(χ^(2)=7.800,5.763,8.453,9.938;all P<0.05).The receiver operating cha
