血清miR⁃195⁃5p、miR⁃409⁃3p对胃癌诊断和预后的价值

摘　　要：目的结合基因表达数据库(GEO)寻找胃癌血清潜在的miRNA肿瘤标志物,并研究对胃癌的诊断和预后价值。方法收集2023年1-6月间在连云港市第二人民医院就诊的30例胃癌患者和20名体检健康人的血清,利用GEO2R在线分析工具对GSE86226数据集筛选出差异表达的miRNA,采用实时荧光定量PCR方法对筛选结果经行验证,并绘制受试者工作特征(ROC)曲线分析血清miR⁃195⁃5p、miR⁃409⁃3p水平对胃癌的诊断价值。仙桃学术在线工具对癌症基因组图谱(TCGA)数据库中胃癌组织和癌旁正常组织中miRNA水平、总生存率进行统计。结果GEO2R在线分析工具显示,胃癌患者和健康人存在10个差异表达的miRNA,分别为hsa⁃miR⁃296⁃5p、hsa⁃miR⁃28⁃3p、hsa⁃miR⁃10b⁃5p、hsa⁃miR⁃141⁃3p、hsa⁃miR⁃95、hsa⁃miR⁃146b⁃5p、hsa⁃miR⁃195⁃5p、hsa⁃miR⁃409⁃3p、hsa⁃miR⁃421、hsa⁃miR⁃200a⁃3p,选择miR⁃195⁃5p和miR⁃409⁃3p这两种在胃癌血清中鲜有报道的miRNA做下一步分析。定量结果显示,miR⁃195⁃5p在胃癌患者血清中表达(1.10±0.32)低于健康人(1.40±0.35),差异有统计学意义(t=2.981,P=0.005);miR⁃409⁃3p表达(0.09±0.13)高于健康人(0.02±0.01),差异有统计学意义(t=-2.310,P=0.025);仙桃学术在线工具分析显示,miR⁃195⁃5p在胃癌组织中低表达,miR⁃409⁃3p在胃癌组织中高表达,与血清定量结果一致。miR⁃195⁃5p和miR⁃409⁃3p联合诊断胃癌的曲线下面积(AUC)为0.900,高于miR⁃195⁃5p的0.729,差异有统计学意义(Z=2.765,P=0.006);而与miR⁃409⁃3p(0.816)差异无统计学意义(Z=1.707,P=0.088)。miR⁃409⁃3p高表达患者总生存率低于miR⁃409⁃3p低表达患者,差异有统计学意义[HR=1.40(1.03~1.89),P=0.032]。结论血清miR⁃195⁃5p、miR⁃409⁃3p水平可以作为胃癌患者诊断标志物,miR⁃195⁃5p、miR⁃409⁃3p联合能够提高胃癌诊断能力;其中miR⁃409⁃3p是较好的胃癌预后指标。Objective To search potential miRNA tumor markers in gastric cancer serum by integrating the Gene Expression Omnibus(GEO),and studied their diagnostic and prognostic value for gastric cancer.Methods The serum samples of 30 patients with gastric cancer and 20 healthy subjects were collected from the Second People's Hospital of Lianyungang between January and June 2023.Differentially expressed miRNAs were screened from GSE86226 data set using GEO2R online analysis tool,and the screening results were verified by real⁃time fluorescence quantitative PCR,and the receiver operating characteristic(ROC)curves were drawn to analyze the diagnostic value of serum levels of miR⁃195⁃5p and miR⁃409⁃3p for gastric cancer.The miRNA level and total survival rate in cancer patients and normal tissues adjacent to cancer in The Cancer Genome Atlas(TCGA)database were counted by the Xiantao Academic Online Tool.Results The GEO2R online analysis showed that there were 10 miRNAs that were differentially expressed,which were hsa⁃miR⁃296⁃5p,hsa⁃miR⁃28⁃3p,hsa⁃miR⁃10b⁃5p,hsa⁃miR⁃141⁃3p,hsa⁃miR⁃95,hsa⁃miR⁃146b⁃5p,hsa⁃miR⁃195⁃5p,hsa⁃miR⁃409⁃3p,hsa⁃miR⁃421 and hsa⁃miR⁃200a⁃3p;miR⁃195⁃5p and miR⁃409⁃3p were selected for further analysis.The quantitative analysis results showed that the expression level of miR⁃195⁃5p in the serum of gastric cancer patients(1.10±0.32)was significantly lower than that of the healthy people(1.40±0.35)(t=2.981,P=0.005),and the expression of miR⁃409⁃3p(0.09±0.13)level was significantly higher than that of the healthy people(0.02±0.01)(t=-2.310,P=0.025).The analysis of Xiantao Academic Online Tool showed that miR⁃195⁃5p was low in gastric cancer tissues,and miR⁃409⁃3p was highly expressed in gastric cancer tissues,which were consistent with serum quantitative results.The area under the curve(AUC)of miR⁃195⁃5p and miR⁃409⁃3p for combined diagnosis of gastric cancer was 0.900,which was higher than 0.729 for miR⁃195⁃5
