摘 要:目的通过静息态脑功能磁共振成像探究乙型肝炎相关轻微肝性脑病患者的脑功能连接(FC)。方法选取2021年8月—2024年8月长沙市第一医院放射科乙型肝炎轻微肝性脑病30例作为研究组,另选取同期体检的健康人群35例作为对照组,均给予静息态脑功能磁共振成像,详细记录检查结果,分析两组人群全脑不同脑区FC相关参数、丘脑FC状况、丘脑灰质体积变化区域以及肝性脑病心理测试评分(PHES)量表得分。结果研究组双侧中央后回、双侧楔叶/枕上小叶以及右侧舌回等脑区FC相关参数低于对照组(P<0.05);以左侧丘脑、右侧丘脑为种子点,研究组丘脑FC均弱于对照组;与对照组比较,研究组双侧丘脑灰质体积明显增加(P<0.05);与对照组相比,研究组数字连接试验-A(NCT-A)、数字连接试验-B(NCT-B)、轨迹描绘试验(LTT)、系列点试验(SDT)评分低于对照组,数字符号试验(DST)评分和PHES总评分高于对照组(P<0.05)。结论乙型肝炎轻微肝性脑病患者应用静息态脑功能磁共振成像,充分反映出静息态下背侧丘脑FC与结构损害,包括双侧中央后回、双侧楔叶/枕上小叶、左侧枕中回、左侧盖部额下回以及右侧舌回FC存在异常。Objective To investigate brain functional connectivity in patients with mild hepatitis B-related hepatic encephalopathy using resting-state functional magnetic resonance imaging.Methods From August 2021 to August 2024,30 cases of mild hepatitis Brelated hepatic encephalopathy from the Radiology Department of Changsha First Hospital were selected as the study group,and 35 healthy individuals who underwent physical examination during the same period were selected as the control group.All participants underwent resting-state functional magnetic resonance imaging,and the examination results were recorded in detail.Functional connections(FC)of different brain regions,functional connections of the thalamus,changes in gray matter volume of the thalamus,and scores of the psychometric hepatic encephalopathy score(PHES)were analyzed in the two groups.Results The FC related parameters of brain regions such as the bilateral central posterior gyrus,bilateral cingulate/occipital lobules,and right lingual gyrus in the study group were lower than those in the control group(P<0.05);Taking the left thalamus as the seed point,the functional connectivity of the thalamus in the study group was weaker than that in the control group,including the bilateral thalamus,right hippocampus,and left middle frontal gyrus;Taking the right thalamus as the seed point,the functional connectivity of the thalamus in the study group was weaker than that in the control group,including the bilateral thalamus,right superior marginal gyrus,and right middle cingulate gyrus;Compared with the control group,the study group showed a significant increase in bilateral thalamic gray matter volume(P<0.05);Compared with the control group,the number connec-tion test-A(NCT-A),number connection test-B(NCT-B),line-tracing test(LTT),and signal detection theory(SDT)scores of the study group were lower than those of the control group,while the digit symbol test(DST)and total PHES scores were higher in the study group than in the control group(P<0.05).Conclusion Patients w
