膀胱癌患者血清HER2表达水平与临床病理特征的相关性分析

摘　　要：目的:探讨膀胱癌患者血清人类表皮生长因子受体2(human epidermal growth factor receptor 2,HER2)表达水平与其临床病理特征的相关性。方法:回顾性分析2022年6月—2024年3月因膀胱尿路上皮癌于南京医科大学第一附属医院接受手术治疗的患者的临床病理资料,比较不同临床病理特征间膀胱癌患者血清HER2表达水平差异。并根据欧洲泌尿外科协会非肌层浸润性膀胱癌(non-muscle-invasive bladder cancer, NMIBC)危险程度分组,对NMIBC患者进行评估,比较各危险程度分组间血清HER2表达水平差异。结果:170例患者纳入研究,其中男135例,女35例;中位年龄67.5(14.5)岁,年龄≤70岁、高级别肿瘤、肌层浸润、合并原位癌及合并脉管内癌栓的膀胱癌患者,血清HER2表达水平更高(P<0.05);高危极高危组NMIBC患者血清HER2表达水平显著高于中低危组(P=0.034),血清HER2表达水平区分高危极高危组NMIBC患者的最佳诊断阈值为8.0 ng/mL,特异度为0.803。结论:血清HER2表达水平升高与膀胱癌患者不良临床病理特征密切相关,血清HER2表达水平有助于术前识别高危极高危NMIBC患者,指导治疗方案。Objective:To investigate the relationship between serum HER2expression level and clinicopathological characteristics in patients with bladder cancer.Methods:We retrospectively collected and analysed clinicopathological data of patients who underwent surgical treatment for bladder urothelial carcinoma at First Affiliated Hospital of Nanjing Medical University from June,2022to March,2024.Serum HER2expression levels among bladder cancer patients with different clinicopathological characteristics were compared.Patients with non-muscle invasive bladder cancer(NMIBC)were grouped according to the European Association of Urology NMIBC risk classification,and the differences in serum HER2expression levels among different risk groups were compared.Results:A total of 170patients were included in the study,consisting of 135males and 35females,with a median age of 67.5(14.5)years.Patients under 70years old,those with high-grade tumors,muscle invasion,carcinoma in situ,and vascular invasion had higher serum HER2expression level(P<0.05).Serum HER2expression levels were significantly higher in the high-risk and very high-risk NMIBC groups compared to the low-risk and intermediate-risk groups(P=0.034),and the optimal diagnostic threshold of distinguishing high-risk and very high-risk NMIBC based on serum HER2expression was 8.0ng/mL,with a specificity of 0.803.Conclusion:The elevated serum HER2expression levels are closely related to the unfavorable clinicopathological characteristics of bladder cancer patients.Serum HER2levels may assist in the preoperative identification of high-risk and very high-risk NMIBC patients,thereby guiding treatment strategies.
