视黄醇结合蛋白在预测膀胱癌根治术后肠梗阻发生的临床价值

摘　　要：目的:分析视黄醇结合蛋白在预测膀胱癌根治+回肠通道术患者术后发生肠梗阻的临床价值。方法:回顾性分析2013年9月—2022年10月在陕西省人民医院接受膀胱癌根治+回肠通道术的具有完整资料的67例患者临床数据,按照术后是否发生肠梗阻将患者分为未发生组和发生组,分析膀胱癌根治+回肠通道术后发生肠梗阻的危险因素。结果:67例患者中24例患者发生肠梗阻,发生率为35.82%。单因素分析发现T3~T4期、胆碱酯酶、视黄醇结合蛋白可能是术后发生肠梗阻的影响因素。多因素分析显示T3~T4期、低水平视黄醇结合蛋白是膀胱癌根治+回肠通道术后发生肠梗阻的独立危险因素。受试者工作特征曲线联合约登指数分析发现视黄醇结合蛋白<36.62 mg/L是术后发生肠梗阻的危险因素。结论:视黄醇结合蛋白水平可能是有效预测膀胱癌根治+回肠通道术后发生肠梗阻的临床指标。Objective:To analyze the clinical value of retinol-binding protein in predicting postoperative ileus in patients with radical cystectomy and ileal conduit.Methods:The clinical data of 67patients who underwent radical cystectomy and ileal conduit in Shaanxi Provincial People's Hospital from September 2013to October 2022were retrospectively analyzed.According to whether postoperative intestinal ileus occurred,the patients were divided into non-occurrence group and occurrence group,and the risk factors in postoperative ileus in patients with radical cystectomy and ileal conduit were analyzed.Results:Among 67patients,24patients developed ileus,and the incidence rate was 35.82%.Univariate analysis showed that T3-T4,cholinesterase and retinol binding protein may be the influencing factors in postoperative ileus.Multivariate analysis showed that T3-T4staging and low level of retinol binding protein were independent risk factors in postoperative ileus in patients with radical cystectomy and ileal conduit.ROC curve combined with Youden index analysis showed that retinol binding protein<36.62mg/L was a risk factor in postoperative intestinal obstruction.Conclusion:Retinol binding protein level may be an effective clinical index to predict postoperative ileus in patients with radical cystectomy and ileal conduit.
