摘 要:目的:分析机器人辅助立体定向微创手术用于治疗高血压脑出血(Hypertensive intracerebral hemorrhage,HICH)的临床价值。方法:回顾性纳入2022年9月~2024年3月在本院接受手术治疗的67例HICH患者,根据治疗方法分为常规立体定向微创手术组(参照组,27例)、机器人辅助立体定向微创手术组(研究组,40例)。观察两组围术期指标、治疗预后及神经功能康复状况、并发症。结果:两组手术时间相比,无统计学意义(P>0.05);研究组血肿清除率、手术费用较参照组高,住院时间较参照组短,差异有统计学意义(P<0.05);研究组患者预后有效率90.00%高于参照组的70.37%,神经功能康复分级优于参照组,差异有统计学意义(P<0.05);研究组并发症发生率2.50%,较参照组的18.51%低,差异有统计学意义(P<0.05)。结论:机器人辅助立体定向微创治疗HICH效果优于常规立体定向微创方法,能改善预后,促进神经功能康复,减少并发症,但会增加手术费用。Objective:To analyze the clinical value of robot-assisted stereotactic minimally invasive surgery in the treatment of hypertensive intracerebral hemorrhage(HICH).Methods:Retrospective selection of research objects,67 patients with HICH were included,all of whom were operated in our hospital from September 2022 to March 2024.According to the treatment methods,they were divided into conventional stereotactic minimally invasive surgery group(reference group,27 cases)and robot-assisted stereotactic minimally invasive surgery group(study group,40 cases).Observe the perioperative indicators,treatment prognosis,neurological rehabilitation and complications status.Results:Operation time between groups was no significant difference(P>0.05).The hematoma clearance rate and operation cost of study group were higher than reference group,and the hospitalization time was shorter than reference group,with statistical significance(P<0.05).The effective rate of prognosis in the study group was 90.00%,which was higher than reference group(70.37%),and the neurological rehabilitation grade was better than that in the reference group(P<0.05).The total complication rate in the study group was 2.50%,which was lower than that in the reference group(18.51%),and the difference was statistically significant(P<0.05).Conclusion:Robot-assisted stereotactic minimally invasive treatment of HICH is better than conventional stereotactic minimally invasive treatment,which can improve prognosis,promote neurological rehabilitation and reduce complications,but it will increase the cost of surgery.
