膀胱癌组织中成纤维细胞生长因子受体3、细胞周期蛋白依赖性激酶抑制剂2A表达与患者临床病理特征和复发的关系

摘　　要：目的探讨膀胱癌组织中成纤维细胞生长因子受体3(FGFR3)、细胞周期蛋白依赖性激酶抑制剂2A(CDKN2A)表达情况及其与患者临床病理特征和复发的关系。方法选择2021年3月至2023年3月驻马店市中心医院收治的膀胱癌患者115例为研究对象,患者入组后均接受综合规范化治疗,采用免疫组织化学法检测膀胱癌组织中FGFR3、CDKN2A蛋白表达情况,统计临床病理特征,采用Spearman相关性分析膀胱癌组织中FGFR3、CDKN2A蛋白表达与患者临床病理特征的相关性。对115例患者进行为期1 a随访,观察复发情况,依据复发情况将患者分为复发组(n=41)和良好组(n=74),采用单因素、多因素logistic回归方程分析膀胱癌组织中FGFR3、CDKN2A蛋白表达情况与膀胱癌复发的关系。结果膀胱癌组织中FGFR3、CDKN2A蛋白表达与病理类型无关(P>0.05);与浸润深度、淋巴结转移、远处转移、TNM分期、分化程度有关(P<0.05);Spearman相关性分析,膀胱癌病理特征中病理类型与膀胱癌组织中FGFR3、CDKN2A蛋白表达情况无显著相关性(r=0.206、-0.285,P>0.05);浸润深度、淋巴结转移、远处转移、TNM分期、分化程度与膀胱癌组织中FGFR3蛋白表达情况呈正相关(r=0.533、0.537、0.561、0.612、0.625,P<0.05),与CDKN2A蛋白表达情况呈负相关(r=-0.591、-0.529、-0.542、-0.617、-0.625,P<0.05)。多因素分析结果显示,FGFR3高表达、CDKN2A低表达、浸润深度T 4、淋巴结转移N 2、远处转移M 1、TNM分期Ⅳ期、分化程度低分化是影响膀胱癌患者复发的独立危险因素(比值比=3.758、2.751、3.370、3.688、3.294、3.823、3.281,P<0.05)。结论膀胱癌组织中FGFR3、CDKN2A表达异常,且与临床病理特征呈显著相关性。FGFR3高表达、CDKN2A低表达是影响膀胱癌复发的危险因素,临床中可早期监测,以掌握患者病情。Objective To investigate the expression of fibroblast growth factor receptor 3(FGFR3)and cyclin-dependent kinase inhibitor 2A(CDKN2A)in bladder cancer tissues and their relationship with clinicopathological characteristics and recurrence of patients with bladder cancer.Methods A total of 115 patients with bladder cancer admitted to Zhumadian Central Hospital from March 2021 to March 2023 were selected as the study subjects.All of them received comprehensive standardized treatment after admission.The expression of FGFR3 and CDKN2A in bladder cancer tissues was detected by immunohistochemistry(IHC),and the clinicopathological characteristics were analyzed.The correlation between the expression of FGFR3 and CDKN2A in bladder cancer tissues and the clinicopathological characteristics of patients with bladder cancer was explored by Spearman correlation analysis.In addition,115 patients were followed up for 1 year to observe the recurrence of bladder cancer.Based on the recurrence status,they were divided into the recurrence group(n=41)and the non-recurrence group(n=74).Univariate and multivariate logistic regression equations were used to analyze the correlation between the expression of FGFR3 and CDKN2A in bladder cancer tissues and the recurrence of this cancer.Results The expression of FGFR3 and CDKN2A proteins in bladder cancer was not correlated with pathological type(P>0.05),but correlated with the depth of invasion,lymph node metastasis,distant metastasis,TNM stage,and differentiation degree(P<0.05).Spearman correlation analysis showed that there was no significant correlation between the pathological type and the expression of FGFR3 and CDKN2A in bladder cancer tissues(r=0.206 and-0.285,P>0.05).The depth of invasion,lymph node metastasis,distant metastasis,TNM stage,and differentiation degree were positively correlated with the expression of FGFR3(r=0.533,0.537,0.561,0.612,and 0.625,P<0.05)but negatively correlated with the expression of CDKN2A(r=-0.591,-0.529,-0.542,-0.617,and-0.625,P<0.05)in bladder cancer t
