摘 要:目的探讨抑铁素2(LCN2)在溃疡性结肠炎(UC)病变组织中的表达及其差异表达细胞的种类。方法收集首都医科大学附属北京朝阳医院消化内科2018年3—9月收治的4例3个月内未接受过治疗的活动期左半结肠型UC患者的病变部位组织和右半结肠正常黏膜组织(自身对照),以及4例健康对照的正常肠黏膜组织(健康对照),采用免疫组化染色观察组织中LCN2的表达情况,单细胞测序技术鉴定LCN2差异表达的细胞类型。结果免疫组化染色结果显示,LCN2在UC病变组织中的表达显著高于自身对照和健康对照。单细胞测序结果显示,与自身对照及健康对照比较,LCN2均是UC病变组织中肠上皮细胞、肠上皮细胞祖细胞、杯状细胞、杯状细胞祖细胞及LGR5+干细胞共有的差异表达基因,且杯状细胞、杯状细胞祖细胞及LGR5+干细胞中LCN2的表达差异更加明显。结论LCN2介导的上皮细胞和杯状细胞损伤及其修复缺陷可能参与了UC的肠黏膜屏障损伤。Objective To investigate the expression of lipocalin-2(LCN2)in ulcerative colitis(UC)lesion tissues and the types of cells with differential expression.Method Tissue samples were collected from four patients with active left-sided UC who had not received treatment within three months,admitted to the Department of Gastroenterology at Beijing Chao-yang Hospital,Capital Medical University,between March and September 2019.Samples included lesion tissues from the left colon,normal mucosal tissues from the right colon(self-controls,SC),and normal intestinal mucosal tissues from four healthy controls(HC).Immunohistochemical staining was used to observe LCN2 expression in tissues,and single-cell sequencing was employed to identify the cell types with differential LCN2 expression.Result Immunohistochemical staining showed significantly higher LCN2 expression in UC lesion tissues compared to both self-controls and healthy controls.Single-cell sequencing revealed that LCN2 was a commonly differentially expressed gene in intestinal epithelial cells,intestinal epithelial progenitor cells,goblet cells,goblet cell progenitor cells,and LGR5+stem cells in UC lesion tissues,regardless of comparison with self-controls or healthy controls.The differential expression of LCN2 was more pronounced in goblet cells,goblet cell progenitor cells,and LGR5+stem cells.Conclusion LCN2-mediated epithelial and goblet cell damage,along with impaired repair mechanisms,may contribute to intestinal mucosal barrier injury in UC.
