血清SCC、CEA、CA125、CYFRA21-1水平与食管癌患者病理特征的相关性及诊断效能  

Correlations between serum SCC,CEA,CA125,CYFRA21-1 levels and pathological features of esophageal cancer patients and their diagnostic efficacy

摘  要:目的:分析血清鳞状细胞癌抗原(SCC)、癌胚抗原(CEA)、糖类抗原125(CA125)、细胞角质蛋白19片段抗原(CYFRA21-1)水平与食管癌患者病理特征的相关性及诊断效能。方法:回顾性分析2020年10月至2022年10月该院收治的60例食管癌患者的临床资料,设为观察组,另选取同期60名健康体检者,设为对照组。比较两组及不同病理特征患者的血清SCC、CEA、CA125、CYFRA21-1水平,采用Pearson相关性分析血清SCC、CEA、CA125、CYFRA21-1水平与食管癌患者病理特征的相关性,并采用受试者工作特征(ROC)曲线分析血清SCC、CEA、CA125、CYFRA21-1单项及联合检测在食管癌中的诊断效能。结果:观察组SCC、CEA、CA125、CYFRA21-1水平均高于对照组,差异有统计学意义(P<0.05);中低分化、临床分期Ⅲ~Ⅳ期、有淋巴结转移的食管癌患者血清SCC、CEA、CA125、CYFRA21-1水平均高于高分化、临床分期Ⅰ~Ⅱ期、无淋巴结转移的食管癌患者,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,血清SCC、CEA、CA125及CYFRA21-1水平与食管癌患者的肿瘤分化程度呈负相关(r<0,P<0.05),与肿瘤临床分期、淋巴结转移均呈正相关(r>0,P<0.05);ROC曲线结果显示,血清SCC、CEA、CA125及CYFRA21-1水平单项及联合检测诊断食管癌的曲线下面积分别为0.743、0.780、0.611、0.760、0.901,其中联合检测诊断效能最高。结论:血清SCC、CEA、CA125、CYFRA21-1水平与食管癌患者的肿瘤分化程度呈负相关,与肿瘤临床分期和淋巴结转移均呈正相关,且联合检测诊断食管癌的效能高于四者单项检测。Objective:To analyze correlations among squamous cell carcinoma antigen(SCC),carcinoembryonic antigen(CEA),carbohydrate antigen 125(CA125)and cyto-keratin 19 fragment antigen 21-1(CYFRA21-1)levels in patients with esophageal cancer and pathological features and their diagnostic efficiencies.Methods:The clinical data of 60 patients with esophageal cancer admitted to this hospital from October 2020 to October 2022 were retrospectively analyzed and they were set as the observation group,and 60 healthy subjects were selected as the control group.The levels of serum SCC,CEA,CA125 and CYFRA21-1 were compared between the two groups and among the patients with different pathological features.Pearson correlation analysis was used to analyze the correlations between the serum SCC,CEA,CA125,CYFRA21-1 levels and pathological features of patients with esophageal cancer.The receiver operating characteristic(ROC)curve was used to analyze the diagnostic efficacy of single and combined detection of serum SCC,CEA,CA125 and CYFRA21-1 in esophageal cancer.Results:The levels of SCC,CEA,CA125 and CYFRA21-1 in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of serum SCC,CEA,CA125 and CYFRA21-1 in the patients with middle and low differentiation,clinical stage III-IV and lymph node metastasis were higher than those in the patients with high differentiation,clinical stage I-II and no lymph node metastasis,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the levels of serum SCC,CEA,CA125 and CYFRA21-1 were negatively correlated with the degree of tumor differentiation in the patients with esophageal cancer(r<0,P<0.05),and positively correlated with clinical stage and lymph node metastasis(r>0,P<0.05).ROC curve results showed that the area under the curve of single and combined detection of serum SCC,CEA,CA125 and CYFRA21-1 levels in the diagnosis of esophageal cancer was 0.743,0.780,0.611,0.760 and 0.9
