摘 要:目的分析在急性脑出血诊断中应用扩散加权成像联合磁敏感加权成像高场强MRI诊断的价值。方法回顾性收集泉州市医学高等专科学校附属人民医院急诊科于2021年1月至2023年12月收治的疑似急性脑出血的364例患者临床资料,以结合临床表现以及颅脑CT见有出血病灶为金标准,分析扩散加权成像联合磁敏感加权成像高场强MRI及单一诊断的诊断结果。结果扩散加权成像联合磁敏感加权成像高场强MRI诊断急性脑出血,阳性例数更接近金标准,Kappa值高度一致性,且检出符合率高达97.00%(P<0.05)。在两项序列的影像学特征上,扩散加权成像以血肿区域混杂信号为主,而血肿周围信号为高信号,且血肿边缘存在有线状信号边缘清楚,而磁敏感加权成像高场强MRI同样血肿边缘清楚、血肿周围信号特征呈高信号,但在血肿中心区域部分以低信号为主。结论急性脑出血患者诊断中应用扩散加权成像联合磁敏感加权成像高场强MRI有较高的阳性检出率,能够展示出血病灶部位的影像学特征,利于临床鉴别出血面积和周围累及情况,从而为手术以及药物治疗提供有效的影像学数据支持。Objective To analyze the value of diffusion-weighted imaging(DWI)combined with magnetic susceptibility-weighted imaging(SWI)high-field MRI in the diagnosis of acute cerebral hemorrhage.Methods A retrospective study was conducted to collect the clinical data of 364 suspected acute cerebral hemorrhage patients admitted to the emergency department of Affiliated People’s Hospital of Quanzhou Medical College from January 2021 to December 2023,and the gold standard was defined as the combination of clinical manifestations and intracranial CT findings,analysis of the diagnostic results of diffusion-weighted imaging combined with magnetic susceptibility weighted imaging and single diagnosis.Results The combined use of DWI and SWI high-field MRI in the diagnosis of acute cerebral hemorrhage resulted in a higher number of positive cases that were closer to the gold standard,Kappa values are highly consistent,with a detection consistency of up to 97%(P<0.05).In terms of imaging features of the two sequences,the DWI sequence showed mixed signals in the hematoma area,while the surrounding signals were high.The hematoma edge had a clear linear signal edge,while the SWI sequence also showed a clear hematoma edge and high signals around the hematoma.However,the central area of the hematoma was characterized by low signals in some cases.Conclusions The combined use of DWI and SWI high-field MRI has a high positive detection rate in the diagnosis of acute cerebral hemorrhage,which can display the imaging features of the hemorrhage lesion and facilitate clinical differentiation of the hemorrhage area and surrounding involvement,thereby providing effective imaging data support for surgical and drug therapy.
