基于脾肾阳虚探讨化疗相关性腹泻的辨治

摘　　要：化疗相关性腹泻是接受化疗癌症患者最常出现的临床并发症之一,影响患者的身心健康,导致生活质量下降并阻碍后续治疗。西医对化疗相关性腹泻治疗往往有恶心呕吐等不良反应。中医药治疗毒副作用低且遵循治病求本、标本兼顾等原则,对本病的治疗疗效显著。化疗相关性腹泻已经逐渐成为中医药优势病种之一,属于中医“泄泻”等范畴,其病机多为脾虚湿盛,临床多从湿邪探讨其病机与治疗。而根据本病对象为化疗患者,久病多虚,伤及脏腑阳气,认为治疗应从脾肾论治,温肾补脾贯穿治疗过程。Chemotherapy-induced diarrhea is one of the most common clinical complications in cancer patients receiving chemotherapy,and affects the physical and mental health of patients,leading to reduced quality of life and hindrant follow-up treatment.However,Western medicine treats Chemotherapy-induced diarrhea in a single way,with more symptomatic treatment and poor curative effect.Traditional Chinese medicine has low toxic and side effects and follows the principle of treating the disease by seeking the root and taking into account the specimen.Chemotherapy-induced diarrhea has gradually become one of the dominant diseases of traditional Chinese medicine,which belongs to “diarrhea” in traditional Chinese medicine,etc.Its pathogenesis is mostly spleen deficiency and dampness,and its pathogenesis and treatment are mostly discussed from dampness pathogenic factors in clinic.According to the object of this disease for chemotherapy patients,chronic illness and deficiency,injury to viscera are mainly closely related to the spleen and kidney two viscera,it is believed that the treatment should be based on spleen and kidney.This paper discusses the relationship between Chemotherapy-induced diarrhea and spleen and considers that warming kidney and invigorating spleen run through the treatment process.
