慢性萎缩性胃炎患者血清ProGRP、CA242、PGⅠ、PGⅡ、表达及其与Hp感染的关系

摘　　要：目的探讨慢性萎缩性胃炎(CAG)患者血清糖链抗原242(CA242)、胃泌素释放肽前体(ProGRP)、胃蛋白酶原(PG)Ⅰ、PGⅡ表达及其与幽门螺杆菌(Hp)感染的关系。方法选择92例CAG患者为观察组,选择同期诊治的慢性非萎缩性胃炎患者(117例)为对照组。比较两组血清ProGRP、CA242、PGⅠ、PGⅡ水平,并采用受试者工作特征(ROC)曲线分析ProGRP、CA242、PGⅠ、PGⅡ诊断CAG的诊断价值。根据呼气试验判断是否Hp感染,并分为Hp感染组和非感染组,分析比较Hp感染组与非感染组的血清ProGRP、CA242、PGⅠ、PGⅡ水平差异。结果观察组患者的血清ProGRP、CA242水平高于对照组,血清PGⅠ、PGⅡ水平低于对照组,差异均有统计学意义(t分别=6.85、9.19、-20.38、-10.16,P均<0.05)。ProGRP、CA242、PGⅠ、PGⅡ及联合检测诊断CAG的曲线下面积(AUC)分别为0.67、0.72、0.74、0.69、0.91,灵敏度分别为63.04%、70.65%、71.74%、71.74%、80.43%,特异度分别为71.19%、70.94%、72.65%、63.25%、85.47%,其中联合检测的AUC高于ProGRP、CA242、PGⅠ、PGⅡ单一检测的AUC(Z分别=5.61、4.70、4.37、5.01,P均<0.05)。根据呼气试验结果,28例呼气试验阳性的患者纳入Hp感染组,其余64例患者纳入非感染组,Hp感染组血清ProGRP、CA242水平均高于非感染组,血清PGⅠ、PGⅡ水平低于非感染组,差异均有统计学意义(t分别=8.97、9.19、-16.91、-3.29,P均<0.05)。结论ProGRP、CA242、PGⅠ、PGⅡ对CAG的诊断具有重要指导意义,其中联合检测的诊断价值最高,且ProGRP、CA242、PGⅠ、PGⅡ水平与Hp感染具有密切关系。Objective To explore the serum carbohydrate antigen 242(CA242),gastrin releasing peptide precursor(ProGRP),pepsinogen(PG)Ⅰ,PGⅡin patients with chronic atrophic gastritis(CAG)and their relationship with helicobacter pylori(Hp)infection.Methods A total of 92 patients with CAG were selected as the observation group,and 117 patients with chronic non atrophic gastritis treated during the same period were selected as the control group.The serum levels of ProGRP,CA242,PGⅠand PGⅡin the two groups were compared,and the diagnostic value of ProGRP,CA242,PGⅠand PGⅡin the diagnosis of CAG was analyzed by receiver operating characteristic(ROC)curve.The serum levels of ProGRP,CA242,PGⅠand PGⅡbetween the Hp infection group and the non-infection group were analyzed and compared.Results The levels of serum ProGRP and CA242 in the observation group were higher than the control group,while the levels of serum PGⅠand PGⅡwere lower than the control group,the difference was statistically significant(t=6.85,9.19,-20.38,-10.16,P<0.05).The area under the curve(AUC)of ProGRP,CA242,PGⅠ,PGⅡ,and combined detection for the diagnosis of CAG were 0.67,0.72,0.74,0.69,and 0.91,respectively.The sensitivity were 63.04%,70.65%,71.74%,71.74%,80.43%,and the specificity were 71.19%,70.94%,72.65%,63.25%,and 85.47%,respectively.The AUC of joint detection was higher than that of single detection of ProGRP,CA242,PGⅠ,and PGⅡ(Z=5.61,4.70,4.37,5.01,P<0.05).According to the breath test results,28 patients with positive breath tests were included in the Hp infection group,while the remaining 64 patients were included in the non infection group.The serum levels of ProGRP and CA242 in the Hp infected group were higher than those in the non infected group,the serum levels of PGⅠand PGⅡwere lower than those in the non infected group,the difference is statistically significant(t=8.97,9.19,-16.91,-3.29,P<0.05).Conclusion ProGRP,CA242,PGⅠand PGⅡare of great significance in the diagnosis of CAG,and ProGRP,CA242,PGⅠand PGⅡlevels ar
