固肾益智汤联合多巴丝肼治疗血管性帕金森综合征的有效性及安全性观察

摘　　要：目的观察固肾益智汤联合多巴丝肼治疗血管性帕金森综合征(VP)患者的有效性及安全性。方法选取2022年1月至2024年1月北京中医医院怀柔医院脑病科收治的80例VP患者,按照随机数字表法分为对照组和观察组,每组40例。对照组采用多巴丝肼治疗,观察组采用固肾益智汤联合多巴丝肼治疗,治疗8周。比较2组治疗效果、临床症状[帕金森病统一评分量表(UPDRS)]、脑神经营养因子[神经生长因子(NGF)、脑源性神经营养因子(BDNF)]、血液流变学指标[全血黏度(高切)(HS)、全血黏度(低切)(LS)、血浆黏度(PV)、血小板聚集率(PAA)]及不良反应。结果观察组总有效率(90.00%)较对照组(70.00%)高(P<0.05)。治疗8周,2组UPDRS各项评分和HS、LS、PV、PAA水平均降低(P<0.05),且观察组UPDRS各项评分和HS、LS、PV、PAA水平较对照组低(P<0.05)。治疗8周,2组NGF、BDNF水平均升高(P<0.05),且观察组NGF、BDNF水平较对照组高(P<0.05)。2组不良反应比较差异无统计学意义(P>0.05)。结论固肾益智汤联合多巴丝肼治疗VP患者有效率高,可减轻临床症状,调节脑神经生长因子,改善血液流变学,且安全性好。Objective To evaluate the efficacy and safety of Gushen Yizhi Decoction combined with Madopar in the treatment of vascular Parkinson's syndrome(VP).Methods A total of 80 VP patients admitted to the Department of Neurology,Beijing Traditional Chinese Medicine Hospital,Huairou Branch,from January 2022 to January 2024 were included.Patients were randomly divided into a control group(40 cases)and an observation group(40 cases)using the random number table method.The control group received Madopar monotherapy,while the observation group received Gushen Yizhi Decoction combined with Madopar therapy.Both groups were treated for 8 weeks.The treatment outcomes,including clinical symptoms[Unified Parkinson's Disease Rating Scale(UPDRS)],cerebral neurotrophic factors[nerve growth factor(NGF),brain-derived neurotrophic factor(BDNF)],blood rheological parameters[whole blood highshear viscosity(HS),whole blood low-shear viscosity(LS),plasma viscosity(PV),platelet aggregation rate(PAA)],and adverse reactions,were compared between the groups.Results The observation group demonstrated a significantly higher total effective rate(90.00%)compared to the control group(70.00%)(P<0.05).After 8 weeks of treatment,the UPDRS scores,HS,LS,PV,and PAA levels in both groups significantly decreased(P<0.05),with the observation group showing lower values compared to the control group(P<0.05).Meanwhile,NGF and BDNF levels in both groups significantly increased(P<0.05),with the observation group exhibiting higher levels than the control group(P<0.05).No statistically significant difference was observed in the incidence of adverse reactions between the two groups(P>0.05).Conclusion GushenYizhi Decoction combined with Madopar is effective for treating VP,alleviating clinical symptoms,enhancing cerebral neurotrophic factor levels,improving blood rheology,and exhibiting good safety.
