儿童良性癫痫伴中央颞区棘波的长程脑电图特点分析

摘　　要：目的分析儿童良性癫痫伴中央颞区棘波的长程脑电图特点。方法选取于江苏省扬州五台山医院门诊以及住院部2018年3月至2024年3月接受治疗的52例良性癫痫伴中央颞区棘波患儿,依据发作间期患儿睡眠期痫性波数量不同分为轻度组(偶见、少量、较多)40例,中重度组(中至多量及频发)12例,比较两组的发作类型、抗癫痫发作药物使用情况。结果轻度组的局灶发作占比高于中重度组,局灶性继发全面性发作占比低于中重度组(P<0.05);轻度组的联合用药占比低于中重度组,未治疗与单药治疗占比高于中重度组(P<0.05)。结论中轻度良性癫痫伴中央颞区棘波患儿的长程脑电图以局灶发作为主,中重度以局灶性继发全面性发作为主;轻度可无需治疗或者单药治疗,而中重度者则以联合用药为宜。Objective To analyze the long range electroencephalogram characteristics of benign epilepsy of children with centro-temporal spike.Methods A total of 52 children with benign epilepsy with centro-temporal spike treated in the Department of Outpatient and Inpatient,Wutaishan Hospital of Yangzhou,Jiangsu from March 2018 to March 2024 were selected and divided into mild group(occasional,few and more epileptic waves)with 40 cases and moderate and severe group(moderate to maximum amount and frequent epileptic waves)with 12 cases according to the number of epileptic waves in the sleep period of the children at interparoxysmal period.The seizure type and the use of antiepileptic drugs were compared between the two groups.Results The proportion of focal seizure in the mild group was higher than that in the moderate and severe group,and the proportion of focal secondary generalized seizure was lower than that in the moderate and severe group(P<0.05).The proportion of combined drug use in the mild group was lower than that in the moderate and severe group(P<0.05),while the proportion of nontreatment and monotherapy was higher than that in the moderate and severe group(P<0.05).Conclusion In the long range electroencephalogram of benign epilepsy of childhood with centro-temporal spike,mild cases are mainly characterized by focal seizure,while moderate and severe cases are mainly characterized by focal secondary generalized seizure.Mild cases may not require treatment or monotherapy,while moderate to severe cases should be treated with monotherapy.
