摘  要:目的采用低频振幅(ALFF)联合功能连接(FC)对遗忘型轻度认知障碍(aMCI)伴高血压患者进行研究,以探讨aMCI伴高血压患者脑功能异常情况。方法选取51例aMCI伴高血压患者(研究组)和52名年龄、性别、教育年限、民族相匹配的健康对照(对照组),进行神经与其他各项心理评估及静息态功能磁共振扫描。采用DPARSFA 2.3和SPM 12软件分析筛选ALFF异常的脑区,选取ALFF生成的楔前叶(42,-12,63)为兴趣区进行全脑功能连接FC。结果两组蒙特利尔认知评估基础量表中文版(MoCA-B)评分、阿尔茨海默病评估量表-认知量表(ADAS-Cog)评分、收缩压、舒张压比较,差异均有统计学意义(P<0.05)。与对照组相比,研究组双侧额中回、右侧楔前叶ALFF活动增强(P<0.001);研究组楔前叶与右侧中央前回、双侧中央后回、右侧梭状回功能连接增强(P<0.001)。结论默认模式网络(DMN)功能活动异常可能与AD早期的病理机制有关,额中回、楔前叶功能活动异常可能参与了aMCI伴高血压患者认知功能改变的神经机制。Objective To explore the brain function abnormalities in patients with amnestic mild cognitive impairment(aMCI)with hypertension through amplitude of low-frequency fluctuation(ALFF)combined with whole brain functional connectivity(FC).Methods A total of 51 patients with aMCI combined with hypertension(study group)and 52 age,gender,education and nationality-matched healthy controls(control group)were included and underwent neuropsychological evaluations and resting-state functional magnetic resonance imaging scanning.Abnormal brain areas in study group were screened and analyzed through DPARSFA2.3 and SPM12 software programs.A whole-brain FC analysis was conducted with the precuneus(42,-12,63)as the region of interest.Results There existed significant differences in Montreal Cognitive Assessment-Basic Scale(MoCA-B)and Alzheimer’s Disease Assessment Scale-Cognitive Scale(ADAS-Cog)scores,SBP,DBP between two groups(P<0.05).Compared with control group,study group displayed higher ALFF in bilateral middle frontal gyrus,right precuneus(P<0.001).The whole brain FCs in study group increased between right precuneus with right precentral gyrus,bilateral postcentral gyrus,right fusiform gyrus(P<0.001).Conclusion The abnormalities in default model network may indicate the early-stage pathological mechanism of AD.The functional alternations in middle frontal gyrus and precuneus may play an important role in the neural mechanism of cognitive changes in patients with aMCI combined with hypertension.
