摘 要:目的 探讨主动循环呼吸训练联合有效排痰管理在肺癌围手术期患者康复中的应用效果。方法 选取2023年1—12月于昆山市第一人民医院接受手术治疗的102例围手术期肺癌患者,采用掷骰子方法分为联合组与常规组各51例。常规组给予常规护理,联合组在常规组护理基础上,给予主动循环呼吸训练联合有效排痰管理。两组患者均观察5 d,记录术后首次咳痰时间、术后住院时间、胸导管留置时间、呼气流量峰值(PEF)、第1秒用力呼气容积(FEV1),以及并发症发生情况。结果 与常规组比较,联合组术后住院时间、胸导管留置时间均缩短,每日排痰量增加,术后首次咳痰时间减少,差异有统计学意义(P<0.05)。护理5 d后,两组FEV1与PEF高于护理前,且联合组高于常规组,差异有统计学意义(P<0.05)。护理期间,联合组并发症发生率低于常规组,差异有统计学意义(P<0.05)。结论 肺癌围手术期患者采用主动循环呼吸训练联合有效排痰管理,能缩短术后住院时间、胸导管留置时间,增加每日排痰量,还可加快术后首次咳痰时间,有利于改善患者的肺功能,降低患者的并发症发生率。Objective To explore the application effect of active circulatory breathing training combined with effective sputum management in the rehabilitation of perioperative lung cancer patients.Methods A total of 102 perioperative lung cancer patients who underwent surgical treatment at Kunshan First People's Hospital from January to December 2023 were selected and divided into a combination group and a conventional group by a dice rolling method,with 51 cases in each group.The routine group received routine care,while the combined group received active circulatory breathing training combined with effective sputum management on the basis of routine care.Both groups of patients were observed for 5 days,and the time of first sputum production after surgery,length of hospital stay after surgery,duration of chest catheterization,peak expiratory flow(PEF),forced expiratory volume in one second(FEV1),and incidence of complications were recorded.Results Compared with the conventional group,the combined group showed a decrease in postoperative hospitalization time and chest catheterization retention time,an increase in daily sputum output,and a decrease in the first sputum cough time after surgery,with statistical significance(P<0.05).After 5 days of nursing the FEV1 and PEF of the two groups were higher than before nursing,and the combined group was higher than the control group,with statistical significance(P<0.05).During the nursing period,the incidence of complications in the combination group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusions Active circulatory breathing training combined with effective sputum management can shorten postoperative hospitalization time,chest catheterization retention time,increase daily sputum output,accelerate the first sputum production time after surgery,improve lung function,and reduce the incidence of complications in patients with lung cancer during the perioperative period.
