摘 要:目的 探讨胃癌原发灶与淋巴结转移灶中B细胞淋巴瘤因子-2(Bcl-2)、血管内皮生长因子(VEGF)的表达情况,重点分析Bcl-2、VEGF与化疗敏感性的相关性。方法 采用前瞻性研究,纳入82例伴淋巴结转移的胃癌患者作为研究对象,取原发灶组织与淋巴结转移病灶组织,测定原发灶和淋巴结转移灶中Bcl-2、VEGF表达情况,并检测原发灶与淋巴结转移灶的体外化疗药物敏感性(肿瘤抑制率),采用点二列相关检验,分析不同病灶中Bcl-2、VEGF表达与化疗敏感性的相关性。结果 胃癌淋巴结转移灶中Bcl-2、VEGF阳性表达率高于原发灶(P<0.05)。5-氟尿嘧啶、卡培他滨、替吉奥、紫杉醇对原发灶肿瘤细胞抑制率低于淋巴结转移灶(P<0.05),顺铂、奥沙利铂对原发灶肿瘤细胞抑制率高于淋巴结转移灶(P<0.05)。点二列相关性分析显示,胃癌原发灶、淋巴结转移灶的Bcl-2、VEGF表达均与主要化疗药物对肿瘤细胞的抑制率呈负相关(γ<0,P<0.05)。结论 胃癌淋巴结转移灶Bcl-2、VEGF阳性表达率更高,不同病灶对不同化疗药物可表现出化疗敏感性差异,且无论是原发灶还是淋巴结转移灶,其病灶组织中Bcl-2、VEGF表达均与化疗敏感性呈负相关性。Objective To investigate the expression of B-cell lymphoma factor-2(Bcl-2)and vascular endothelial growth factor(VEGF)in primary and lymph node metastases of gastric cancer,with a focus on analyzing the correlation between Bcl-2,VEGF and chemotherapy sensitivity.Methods A prospective study was conducted to include 82 patients with gastric cancer with lymph node metastasis as the research object.The primary tumor tissue and lymph node metastasis tissue were taken to determine the expression of Bcl-2 and VEGF in the primary tumor and lymph node metastasis,and the in vitro chemotherapeutic drug sensitivity test results(tumor inhibition rate)of the primary tumor and lymph node metastasis were detected.The correlation between the expression of Bcl-2 and VEGF in different lesions and the sensitivity of chemotherapy was analyzed by point two column correlation test.Results The positive expression rates of Bcl-2 and VEGF in lymph node metastases of gastric cancer were higher than those in the primary lesion(P<0.05).5-fluorouracil,capecitabine,tigio,and paclitaxel had a lower inhibitory rate on primary tumor cells compared to lymph node metastases(P<0.05),while cisplatin and oxaliplatin have a higher inhibitory rate on primary tumor cells compared to lymph node metastases(P<0.05).Point two column correlation analysis showed that the expression of Bcl-2 and VEGF in the primary lesion and lymph node metastasis of gastric cancer was negatively correlated with the inhibitory rate of major chemotherapy drugs on tumor cells(γ<0,P<0.05).Conclusion The positive expression rates of Bcl-2 and VEGF are higher in lymph node metastases of gastric cancer,and different lesions may show differences in chemotherapy sensitivity to different chemotherapy drugs.Whether it is the primary lesion or lymph node metastasis,the expression of Bcl-2 and VEGF in the lesion tissue is negatively correlated with chemotherapy sensitivity.
