淋巴结坏死对早期鼻咽癌患者的预后影响分析

摘　　要：目的探讨淋巴结坏死对早期无转移鼻咽癌患者的预后影响。方法对纳入的266例T1-2N1M0期患者,在MRI图像上评估是否存在淋巴结坏死。对患者进行为期5年的随访,绘制生存曲线图,通过单因素、多因素分析比较有LNN和无LNN患者5年生存差异。结果在所有患者中共有79例(29.7%)患者存在LNN。有LNN患者EBV DNA拷贝数显著高于无LNN患者。单因素分析可见有LNN患者的5年生存显著差于无LNN的患者。多因素分析显示LNN是早期鼻咽癌患者OS(P=0.002)、RC(P=0.049)、DC(P<0.001)的独立预后因素,且有LNN患者CR率也要显著低于无LNN患者。结论淋巴结坏死会显著影响早期鼻咽癌患者的预后,是早期鼻咽癌患者5年OS、RC、DC的显著不良预后因素。Objective To explore the prognostic effect of lymph node necrosis(LNN)in early non-metastatic nasopharyngeal carcinoma(NPC)patients.Methods The presence of lymph node necrosis on MRI images of 266 patients with T 1-2 N 1M 0 stage was evaluated.The patients were followed up for 5 years,survival curves were plotted,and univariate and multivariate analyses were used to compare the differences in 5-year survival between patients with LNN and without LNN.Results A total of 79(29.7%)patients had LNN in all patients.The EBV DNA load of patients with LNN was significantly higher than that of patients without LNN.Univariate analysis showed that patients with LNN had significantly worse 5-year survival than those without LNN.Multivariate analysis showed that LNN was an independent prognostic factor for OS(P=0.002),RC(P=0.049),and DC(P<0.001).The CR rate of patients with LNN was significantly lower than that of patients without LNN.Conclusion LNN can significantly affect the prognosis of early NPC patients,and is a significant adverse prognostic factor for 5-year OS,RC,and DC in early NPC patients.
