显微镜与内镜下经单侧鼻蝶入路手术对垂体瘤患者应激指标及激素水平影响的比较

摘　　要：目的比较显微镜与内镜下经单侧鼻蝶入路手术对垂体瘤患者的影响。方法选取82例垂体瘤患者,随机分成2组,均41例。对照组行显微镜下经单侧鼻蝶入路手术,观察组行内镜下经单侧鼻蝶入路手术。比较2组手术相关指标、应激反应指标、免疫功能、激素水平、并发症。结果观察组各手术相关指标均优于对照组,并发症发生率低于对照组。观察组术后的各应激反应指标、激素水平、CD8+均低于对照组,CD3+、CD4+高于对照组(P<0.05)。结论相比显微镜手术,内镜下经单侧鼻蝶入路手术肿瘤全切率更高、创伤更小、并发症更少,对垂体瘤患者的应激反应及免疫功能影响更小,且可有效地降低激素水平,值得临床借鉴。Objective To compare the effects of unilateral transsphenoidal surgery under microscope and endoscopy on patients with pituitary tumors.Methods 82 patients with pituitary tumors were randomly divided into2 groups,with 41 cases in each group.The control group underwent surgery via a unilateral nasal butterfly approach under a microscope,while the observation group underwent surgery via an endoscopic unilateral nasal butterfly approach.Compare 2 groups of surgical related indicators,stress response indicators,immune function,hormone levels,and complications.Results All surgical related indicators in the observation group were better than those of the control group,and the incidence of complications was lower than that of the control group;And the postoperative stress response indicators,hormone levels,and CD8+in observation group were lower than those of the control group,while CD3+and CD4+were higher than those of the control group(P<0.05).Conclusion Compared with microscopic surgery,endoscopic transnasal transsphenoidal approach has a higher tumor resection rate,smaller trauma,and fewer complications.It has less impact on stress response and immune function in patients with pituitary tumors,and can effectively reduce hormone levels,which is worthy of clinical reference.
