中低位直肠癌根治术中应用预防性回肠双腔与单腔造口的疗效对比研究

摘　　要：目的 探究中低位直肠癌根治术中应用预防性回肠双腔与单腔造口的应用效果。方法 选择中低位直肠癌根治术患者80例,根据所使用干预方法的不同将其划分为单腔造口组(n=35)以及双腔造口组(n=45),单腔造口组患者接受预防性回肠单腔造口术;双腔造口组患者接受预防性回肠双腔造口术。记录两组Ⅰ期手术、Ⅱ期手术阶段围术期相关指标、两组术后结肠炎严重程度、术后并发症发生情况,并进行比较。结果 全部研究对象均顺利完成手术。两组Ⅰ期手术阶段的手术时长、术中出血量、术后排气时间以及总住院时间比较无明显差异(P>0.05);两组Ⅱ期围手术期的术中出血量、距离Ⅰ期手术的时间差异亦无统计学意义(P>0.05),然而双腔造口组患者的手术时长显著短于单腔造口组(P<0.05);单腔造口组患者术后的结肠炎严重程度相比双腔造口组明显更加严重(P<0.05);与此同时,双腔造口组患者术后并发症发生率显著低于单腔造口组(P<0.05)。结论 预防性回肠双腔造口术应用于中低位直肠癌根治术患者,有助于缩短Ⅱ期手术时间、降低结肠炎发生率以及术后并发症发生率,具有较高的应用价值。Objective To explore the application effect of prophylactic double-lumen and single-lumen ileostomy in radical resection of middle and low rectal cancer.Methods A total of 80 patients undergoing radical resection of middle and low rectal cancer were enrolled.According to different intervention methods,they were divided into single-lumen stomy group(n=35,prophylactic single-lumen ileostomy)and double-lumen stomy group(n=45,prophylactic double-lumen ileostomy).The perioperative indexes in stagesⅠ~Ⅱsurgery,severity of postoperative colitis and postoperative complications in the two groups were recorded and compared.Results All the research objects successfully completed the surgery.During stageⅠsurgery,there was no significant difference in operation time,intraoperative blood loss,postoperative exhaust time or total hospitalization time between the 2 groups(P>0.05).During stageⅡsurgery,there was no significant difference in intraoperative blood loss or interval from stageⅠto stageⅡsurgery between the 2 groups(P>0.05),but operation time in double-lumen stomy group was significantly shorter than that in single-lumen stomy group(P<0.05).The severity of postoperative colitis in single-lumen stomy group was more severe than that in double-lumen stomy group(P<0.05).The incidence of postoperative complications in double-lumen stomy group was significantly lower than that in single-lumen stomy group(P<0.05).Conclusion Prophylactic double-lumen ileostomy is beneficial to shorten stage-Ⅱsurgery time,reduce the incidence of colitis and postoperative complications in patients undergoing radical resection of middle and low rectal cancer,which has high application value.
